KNOW YOUR CLIENT FORM - INDIVIDUAL INVESTORS

Step 1 Complete each section of the form. We review this information to determine if you meet the definition of an
Accredited Investor, and are therefore eligible to purchase our Fund. The specific definitions are listed in Schedule C of the Acorn Subscription form. When completing the form, please pay special attention to the definitions of Financial and Net Assets. Financial Assets include cash, securities and insurance contracts but not real estate holdings. By comparison, Net Assets include all of your assets including real estate minus all of your liabilities. We keep this information on file and review it from time to time to make sure our records are up to date. Please contact us at any time with any questions.

Step 2 Certify identification. You are required to provide certified copies of an original identification document (ID). This
includes a valid birth certificate, drivers license, passport, record of landing or permanent resident card. Certification can be attained as follows: (a) If an Acorn representative has met you in person, a photocopy of an original ID is all that is required. (b) If we have not met you in person, we need your ID verified using the following method: Attestation method: Provide us with a legible photocopy of an ID with an original signature (not a photocopy) by a qualified guarantor. The attestation must be on the photocopy of the document and include the name, profession, address, and signature of the guarantor. In this context, a guarantor has to be an individual engaged in one of the following professions in Canada: dentist, medical doctor, chiropractor; judge, magistrate or a lawyer; a notary (in Quebec) or a notary public; an optometrist or a pharmacist; an accredited public accountant (APA), a chartered accountant (CA), a certified general accountant (CGA), a certified management accountant (CMA), a public accountant (PA) or a registered public accountant (RPA); a professional engineer (P. Eng., in a province other than Quebec) or engineer (Eng. in Quebec); or a veterinarian. Certified ID as well as a cleared cheque or wire payment upon investment satisfies the record keeping and client identification requirements of the Proceeds of Crime (Money Laundering) and Terrorist Financing Act (Canada) when opening a new account.
