Food Vendor Application

The town of Pine Lake would like to thank you for your interest in our 2014 LakeFest.
This event will be held on Saturday, October 4
th
, 10 a.m. to 7 p.m. and Sunday,
October 5
th
, 2014, from 12 to 5 p.m. LakeFest will take place along Lakeshore Dr. in
Pine Lake, GA. This beautiful venue will make a lovely setting for visitors to stroll along
the art vendors while enjoying fine food and music from our world-class stage.

LakeFest is seeking to provide a variety of typical festival food items at reasonable
prices, as well as vegetarian, vegan, and/or gluten free items. The ability to provide
a variety of options strengthens your application with the LakeFest committee.

Fees and Application Requirements:

Fees:
o Snack cart vendors (potentially mobile): $50.00.
o Vendors selling food/non-alcoholic beverages: $120.00.
o Vendors selling food/non-alcoholic/alcoholic beverages: $200.00.
o Vendors selling alcoholic beverages only: $200.00
Fees are due to Pine Lake City Arts by July 31, 2014 and are non-refundable for
accepted food vendors.
All food vendor applicants must submit a valid DeKalb County Board of Health
license. If license expires before LakeFest dates, an updated license must be
submitted at least 30 days before LakeFest in order to be allowed to operate.
All vendors applicants selling alcohol must submit a valid State of Georgia liquor
license and apply for a City of Pine Lake liquor license (see additional application
on LakeFest website; fee to be submitted separately). If the state license expires
before LakeFest dates, an updated license must be submitted at least 30 days
before LakeFest in order to be allowed to operate.
All food/drink vendor applicants must submit a copy of their liability insurance
upon application listing Pine Lake LakeFest as Additional Insured. If insurance
expires before LakeFest dates, an updated license must be submitted at least 30
days before LakeFest in order to be allowed to operate.

By signing this application, I understand and accept that this constitutes a contract
between myself (the business) and the Pine Lake LakeFest 2014. I understand that my
application will not be considered complete, and therefore not evaluated, until
payment is received.

If selected as a food vendor to participate in LakeFest 2014, I understand and will
adhere to all operating rules and procedures listed. Please initial each below.

My booth will be open for business during all hours as set by LakeFest. _____
This is an outdoor event. It is my responsibility to prepare for weather
contingencies. LakeFest determines at what point food vendors may close their
booths. _____
If using a tent, I will attach sufficient weight on all corners of my tent to prevent my
tent from becoming a missile in the event of high winds. I want to protect my
investment, the investment of other vendors around me and protect the safety of
the public, fellow exhibitors, LakeFest staff, etc. _____
If using a tent, I will not stake my tent or any other items in the ground or tie my
tent to trees, shrubs, or other objects. _____
Unloading / set-up times: _____
o Friday, Oct 3, 5 8 p.m., with overnight security onsite provided (risk is
shared)
o Saturday, Oct 4, 7 9:30 am, ready for LakeFest opening at 10 am.
o Sunday, Oct 5, 9 11:30, ready for LakeFest opening at 12 pm.
Expect tear down between 7 8 p.m. on Saturday, and 5 - 6 p.m. on Sunday.
LakeFest is a rain or shine event. _____
I will never drive my vehicle on grass or turf. All wheels of my vehicle will remain on
asphalt or other hard surface at all times. _____
I will pick-up/clean up around my booth space before leaving LakeFest on
Sunday evening. I will follow the rule of "leave it better and cleaner than when I
found it". _____
If I need to withdraw from the LakeFest I will do so in writing prior to September 1,
2014. _____
Food vendors must adhere to DeKalb county board of health standards: Please

Food vendors using grease in their operation, or producing grease by-product,
must dispose of grease in an environmentally-friendly way offsite. Any food vendor
found dumping grease on the ground, or any other area on the event property,
will bear the cost of paying for damage and responsibility of cleaning up the
grease. ______
Access to electricity is limited, and will be granted to early applicants in need.
______
Vendors are responsible for proper safety and health precautions to protect
participants, City of Pine Lake, the public and the property of others. ______

Further, I/Vendor agree to indemnify, defend and forever save and hold harmless LakeFest,
its affiliates, and related entities, and their respective parents, employees, representatives,
tenants, agents, contractors and volunteers, from and against any and all damages, claims,
losses, demands, costs, expenses (including attorneys, fees and costs), obligations, liens,
liabilities, actions and causes of action, threatened or actual, which any one of the
indemnities may suffer or incur arising directly or indirectly out of or in connection with the art
exhibit.

If selling alcohol, your application will not be considered complete until you have been
granted a liquor license by the City of Pine Lake (see link to additional application
