• Please note: TransUnion won’t be able to give you specific reasons why we decided not to open
an account for you.

If you would like to contact TransUnion to receive a free copy of your account history, we’ve provided a
form on the second page of this letter. Please fill out the form completely before you send it to
TransUnion.
