1040X for personal-go through 2007 files organize check on fee (LONG TERM-auditor will do
1040 X when completed with 2008 taxes/audit)

Monthly income statements-generate for properties-unable to locate Barb’s spreadsheet on external

drive, but have files for each and every deposit item for each property as well as copies of the
payments

Get all rebills done prior to FYE 6-30-09; completed July 2008, August 2008, Sept. 2008, Oct. 2008
and November 2008. Supplementals for 2 other months as well. Sent priority mail today

Get all reports sent on all clients prior to 6-10-09-Andre working on this w/therapists- in constant
contact wth Andre/therapist on this. Still need Andre’s , Priscillas’ and Doris’ reports.

File small claims on Mentor/Lakeside if under 10K (sent details to Kramer –attorney )100%
completed

Resolve Lawson on BCBS –spoke w/BCBS and spent long time on phone trying to resolve. End
result; need the letter from Primary Care Physician that referred them to us; Priscilla stated you
sent her message on new client back in Set or Oct. 2008 indicating they had insurance coverage, but
nothing in file for referral. Looking into obtaining referral , predated, from physician as insured is
rully aware that their insurance policy is monitored for care and that no treatments to their child can
be done without obtaining the proper referrals

Copy of all Mayfield documents for court-copied 2007 and 2008 notes

Work w/Clinton on court case-done still ongoing-100% completed on my end

Letter to Jewish on Lakesha Tripplet (revising invoices today and sending today-done) 100%
completed-no returned calls to AP Dept. -

Complete all correction invoices for Lakeside (done at $20.00 for travel)-100% completed

Enter current bills that differ every month, in Chase for payment (done through and including 6-
17-09)
