Schedule of medication: once a month (oral meds and injections)
Incident prior to onset of complaint:
- patient became easily tempered (madali magisog) and peculiar behaviors like aggressiveness
(nangangastigo), wandering (naglilinakat) in the community, loss of appetite and weight were
noted (patient is aware of his changing personality). This happens only in 2000 which is the start
of condition. He would beat those persons who make fun of him, like calling him lurong-lurong
because he seeks consult in psychiatric department. Before (no illness) when somebody makes
fun of him, he will just keep quiet. At present when somebody bullies him, he will just tell them
to stop.
- Patient had his first job in gaisano as maintainance crew
- There was a misunderstanding that occurred between him and his co-worker (engineer). He
interpreted the hand signal of his co-worker as youre dead if you dont finish your work, thus
he was scared and spent overtime on his job. This was the beginning of his condition.
- Records of violence: before Yolanda - his cousin destroyed his plants and out of his anger he
ran after his cousin with a knife on his hand but without the intention to kill, only to scare them.
He was brought to municipal hall and was eventually released because of the help of his
classmate who happened to be a counselor in their barangay.
Wifes name: Marilou Pulga
Medication schedule: 2000-2004: good compliance (1xmonth); comes to clinic not accompanied by
anyone else
2004 Dr. Maglasang changed the schedule of his medication by letting him
come after a month then after 1 and month. Alteration of schedule was done
because patient complained that he feels sleepy after taking medication and by
that time he was a construction worker. This lasted for 5 months only since
patient went to mindanao for a week. Claimed to have drinking spree session
with friends thus symptom of inability to sleep was again noted. He was beaten
by his uncle due to some reason.

Diagnosis: Pasma
Vices: drinking alcohol started 1

year high school (tuba everyday; unable to quantify amount) until 200.
Then resumed when he went to Mindanao on 2004. Does not smoke.
Work: patient felt exhausted from his work in gaisano but was still able to sleep after taking overtime.
His working hours start at 7am and ends at 9pm. Overtime usually lasts until 12 midnight. The longest
duration of his condition (unable to sleep) was 1 week without feeling tired. This was accompanied by
feeling of hot flushes (mapaso ha lawas). In work hes able to take his lunch around 2 or 3pm. He is the
only maintenance crew in the said establishment and was not able to get any monetary compensation
since he worked there for just a week.
Job description: turn on the all the lights of the building, fixes generators, lights, aircons.

Childhood: claimed to have good and well-rested sleep
Educational attainment: electrical technology (2 year course) in QEI
Personal history
- Born in Quezon City; NSVD in cephalic presentation at hospital; was breastfed
- No developmental delays as claimed
- No allergies, falls, seizures; (+) mumps
- Actively plays and interacts with other children
- He is an average learner with VALUES education as favorite subject
- Youngest among 2 sibling
- Eldest sister still alive
- Parents: Rolando Gales and Maria Kristina Gales (Both parents are from Bohol but met in Leyte.
His father died and it was his pregnant mom (hiya na ito an ada sulod han tummy :P) who
arranged the pension in Quezon City)
- Right after his mom gave birth in Quezon City they went back to Leyte. He grew up with his
mom and describes her to be good and disciplinarian. Whenever he commits mistake, his mom
would usually beat him using a broom. This was her way of disciplining them. He has close
relationship with his mother and sibling because whenever he has problem he talks and shares it
with them.
- Primary source of money comes from his fathers pension and grandmothers small business
(nagpapawod and nagsasasa di ak maaram kun ano ito) since his mother doesnt have a job. He
claimed that this is enough to meet the essential ne. eds of the family.
- Father is a soldier and died due to malaria
- In Leyte, they stayed in San Juaquin at the house of his grandmother. Her grandmother also beat
him only when he makes a grave offense.
- Household members: patient, sister, mother, grandmother, sibling of grandmother
- Claimed to have good relationship with all household members
- Elementary school was in San Juaquin and claimed to be friendly, shy, not an achiever.
- High school years was in Assumption Tanaun Leyte; he is a graduate of class 1997; experienced
to be suspend in school because of school rules violation (nagpinanluho hiya han screen para
makagawas). Did not experience courtship and having girlfriend.
- College: was able to finish his 2 year course (electrical technology) in QEI by year 1999. The
course was chosen by his mom and he preferred it as well.
- Had history of drug abuse (syrup) taken once in 3

year high school and 1

year college. Was
influenced by classmates. Felt hot flushes, feeling of floating on air, and unable to sleep. But this
condition where not associated with his present illness as claimed.
- After graduating he was able to get his first job in gaisano by year 2000
- Stressors: exhaustion from work
- Activities to relieve stress: wandering in community, basketball, and planting.
- Recreation activities: aalaga sarapati
- Doesnt go to church and not involve in community activities
- Family member with same condition: alex his second cousin
- He met his wife in San Juaquin. They were classmates in elementary. They have 2 kids which are
close to him. Wife doesnt have a job and wasnt able to finish school. Sexual relationship was
graded satisfactorily.
- Contraceptive method: withdrawal
- His 2

child has a heart problem an on maintenance medication. She was diagnosed on 2010.
He became problematic at first but he never views it as a burden to the family. He loves his kids
as claimed.
- As a father he wants his kids to finish school.
- Patient has a history of beating his wife but only once as claimed.
- Start to experience courtship in 2000
- First sexual contact was on 2004 with his first girlfriend of 6 months.
- Patient didnt experienced hallucination, illusion, and delusions.
- Patient learned masturbation during 3
rd
year high school.
- Patient has no plans of working as of this time.
- Patient takes a bath 1 every 2 days
