AFFIDAVIT-COMPLAINT
I, GEORGE D, SOLAMO, of legal age, Filipino, married and a resident
of Penaplata, Island Garden City of Samal(IGACOS), after having been sworn
to in accordance with law, hereby depose and say:
That I am retired Senior Fire Officer which prior to my retirement
sometime in 2007, was last assigned at Samal Fire Station;
That I know the person of Sr.Insp Manuel Garcia of GROSS
DISHONESTY and or other appropriate administrative or criminal offense for
making it appear that the fuel consumption of his motor vehicle in the amount
of P 14,667.50 which he withdrew from Samal Island Multipurpose
Cooperative (SIMC) Gasoline Station and ATTEMPTING to have the said
amount paid by local government unit of IGACOS;
That were if not for the vigilance of the Accounting Office of IGACOS,
the local government thereof would have been DEFRAUDED and
FINANCIALLY DAMAGED in the amount of P 14,667.50 brought about by the
DISHONEST conduct of Sr.Insp. Manuel Garcia
That in support of this accusation, attached are the affidavits of FO1
Ronelio V. Tandug, Ricarda M. Miguelles and Lerma D. Enumerables;
That such acts of Sr.Insp. Manuel Garcia which are indeed
DIPLORABLE, SHAMEFUL and UNTHINKABLE, should be condemned and
shall be dealt with by law severely;
That I am executing this affidavit to attest to the truth of the foregoing
and to apprise the authorities concerned of the aforementioned facts so that
Sr. Insp. Manuel Garcia be administratively or criminally prosecuted;

I N F O R M A T I O N

The undersigned Assistant City Prosecutor accuses CRISOLOGO DE
CALOG y BARRIO and RUDY ANTONIO y MASLOG of the crime of
Frustrated Homicide in relation to RA 7610, committed as flows:

That on or about the 21
st
day of July ,2009 in the City of Muntinlupa,
Philippines, and within the jurisdiction of the Honarable Court, the above-
named accused, conspiring, confederating and mutually helping and aiding
one another, armed with stone, with intent to kill, did then and there willfully,
unlawfully and feloniously attack, assault, strike with stone, JUAN PATRICIO
BACOBO y DIOSO, 12 year old minor, in the different parts of the body and
head, continuously punching and hitting his mouth and body, thereby inflicting
upon his person wounds which are fatal, thus performing all the acts his
person wounds which should have produced the crime of homicide, as a
consequence but nevertheless did not produces it by means of a cause
independent of his will, that is due to the timely and able medical attendance
rendered which prevented his death, such acts of child abuse are prejudicial
to the development and growth of JUAN PATRICIO BABOBO y DIOSO.
