Form for married persons/common law
spouses and accompanying children



This form should be used by persons applying for a residence permit who are married or cohabiting with a person living
in Sweden. The marriage should be registered in your country of origin and in Sweden. Cohabitation must be
confirmed by means of documents showing that you and the referee have been registered at the same address abroad
or in Sweden.

You should enclose this form together with the application form "Application for residence permit to settle in
Sweden, MIGR 161011" when you apply at a Swedish mission abroad.

Appendices
You must enclose:
An extract from the Swedish population register for the referee, a 'family certificate for a
residence permit which confirms the address in the population register, marital status,
children etc. The referee orders this form from the Swedish National Tax Agency and it may
be no more than one month old (if the referee is resident in Sweden).

A copy of your passport containing your personal details.

Extract from your country's population register containing information about marital status
and any children. (You can see what documents are required in your country on the
Swedish Embassys website. You can also contact the Embassy for further information.)

A marriage certificate if you are married.

Copies of judgments governing custody or adoption (where applicable for children who are
making a joint application).

Documents showing that you and the referee have been registered at the same address
abroad or in Sweden (common law spouse).
