UNIT 1
IN THE HOSPITAL
Patient : Good morning, Doctor.

Doctor : Good morning, Mr. Govind, Sit down. You usually drop

destructive food habits. Well go in for all the required tests and start treatment. But as for
now, the nurse in the next room will give you an injection
to reduce your pain and Ill prescribe some medicine for relief (writes the prescription). Keep
off spicy food and drinking for now. Avoid oil. You
have to go slow on these if you need to be fit. Meet me again after two days. I advise full rest
till then.

UNIT II
IN PUBLIC HEALTH CENTER

On friday, the Sedau public health center held the national immunization week. All of
mothers take her baby to there and get the immunization for her baby.
Mrs. Ani is a nurse the get the chance to give injection of vaccines. Mrs. Ani give the
direction to the public to maintain order and waiting the turn with the patient. Before that, she
explain about the purpose about the immunization.

Mrs. Ani : good morning all. Today, Ill give the immunization to your baby. Its have a
purpose to keep the body immune system of our baby. And i hope you all can keep the
maintain order and be patient.

Mrs. Ani start the activity well. Shes not looks fatigue although so many patient come to
her. But, is there something happen. Any patient whom do not want to get the immunization.
Mrs. Ani try to persuade her so the baby can get the injection if the vaccines.

Mrs. Ani : why you dont want to get the injection of vaccines to your baby?
Mrs. Nana : I afraid my child will be sick if he get the injection.
Mrs. Ani : youre wrong. The immunization have the goal to keep the immune system.
So, the disease not easy to attack him.
Mrs. Nana : but usually after get the injection all of the baby must be sick. I worry about it.
Mrs. Ani : dont worry, its just the forms of the body reaction on foreign objects. After a
few days the conditions will be right back normally. So, will you do?
Mrs. Nana : ok, I will do. But, you have to give the medicines that will decrease the fever.
Mrs. Ani : ok, I can do it for you.

After Mrs. Nana agree to give the injection of vaccines. Mrs. Ani direct to do her job.
and finally, the immunization was the end and the events of national immunization week
running cleared.













UNIT III
IN PEDIATRIC WARD
Arnol 2-year-old toddler who suffered from diarrhea, was treated in hospital for 2 days has
not changed.

Doctor : Good morning.
Mrs. ratna : good morning, doc
Nurses : the patient's condition is still weak doc, because his patient

IN PHARMACY

Mrs.Ririn want to redeem the recipe from the doctor.she goes to medisca pharmacy
immediately.
Mrs.Ririn : excuse me sir.
Pharmacist : yes,what can I do for you ?
Mrs.Ririn : I want to redeem the medicines that recipe by
Pharmacist : may I to see the recipe first.
Mrs.Ririn : yes,of course.
(mrs.Ririn take the recipe from her bag and then give it to the pharmacist).
Pharmacist : ok,I will checking the medicines stock first.please, wait in a few
minutes.
Mrs.Ririn : yes, I will.
Then,the pharmacist leave mrs.Ririn to find the medicines needed. Mrs.Ririn waiting with the
patient. After ten minutes past the pharmacist back again with the medicines in his hand.
Pharmacist : mrs.Ririn come here.im sory have been waiting.this is the medicines
you want.
Mrs.Ririn : no problem,thanks for it. How much I pay this medicines to take.
Pharmacist : you just pay twenty five thousand rupiah.
Mrs.Ririn : oh ,ok. Is there any vitamine B Complex here.i forget about
it.can you take it for me,please?
Pharmacist : oh,ok. I will take it.
Mrs. Ririn : thanks,sory about it.
Pharmacist : dont worry. Im fine,please wait again.
After that the pharmacist take the vitamine and find it for mrs.Ririn.
Pharmacist : mrs.Ririn,this is yours.
Mrs.Ririn : ok,thank you.so,how much I have to pay all of the medicines that I
take.
Pharmacist : just added the vitamine.so,you must pay thirty thousand rupiah.
Mrs.Ririn : ok.
( mrs.Ririn pay the medicines and find out the money from her wallet.after that,she leave the
pharmacy and back to home directly).
Mrs.Ririn : this the money I pay with the cash.
Pharmacist : ok.thanks for your cooperate.i hope you will be better soon.
Mrs.Ririn : yes,thanks for your service.nice to meet you.
Pharmacist : nice to meet you too.

Mrs.Ririn gone away and satisfied with the service.she is back with the smiling.

IN THE MEDICAL RECORD
