MEMBERSHIP REQUIREMENTS

FOR COMPOSERS:

I. REGULAR MEMBERS (professionals):
A. FILSCAP Deed of Assignment and Application form
B. Any one (1) of the following

1. One (1) or more songs in commercially-released albums. Composer must provide
FILSCAP with an original CD copy of the album/s in which the songs appear. If the
album is independently produced, applicant must submit a copy of any of the following:
a. Job Order (J.O) from replicator;
b. Permit to replicate from OMB.
c. Proof of consignment to retailer/outlet

2. One (1) or more songs that air regularly on the radio. If song is unreleased, composer
must provide FILSCAP with audio recording of actual radio broadcast of the song or a
certification from the radio station.

3. One (1) or more songs that air regularly on a music video channel. Composer must
provide FILSCAP with a copy of the actual TV broadcast of the song on the music
video channel or a certification from the station.

4. One (1) or more commercial score or jingles, or TV/radio show theme songs (vocal or
instrumental) aired on TV/radio. Composer must provide FILSCAP with a copy of the
actual radio or TV broadcast of the material or a certification from the station.

5. One (1) or more songs performed regularly in a FILSCAP-paying venue. If song is
unreleased, composer must provide FILSCAP with an audio recording of the song
(preferably the actual live performance) plus a written endorsement/certification from
venue regarding performance of the song and artist gig schedule.

6. One (1) or more movie scores. Composer must provide FILSCAP with a video copy of
the finished film.

7. One (1) or more songs sold online at a website. Composer must provide FILSCAP a
copy of the song and information on the website on which the song is being marketed.

C. Notification of Works
D. 1 x 1 ID Picture
E. Tax Identification Number (TIN)
F. Proof of residency (copy of any 2 of the following):
1. Passport (ID Page)
2. Phil. Drivers License
3. SSS ID
4. Voters ID
5. Billing Statement in Applicants Name
6. ITR

II. ASSOCIATE MEMBERS (amateurs)
A. Deed of Assignment and Application form
B. Notification of Works
C. Tapes/ CDs of works
D. 1x1 ID Picture
E. Any Government Issued ID

All Associate membership must notify FILSCAP once their works are recorded or published in order to
be upgraded to regular status. At such time the Associate Member must submit items B, E and F under
REGULAR MEMBERS (I)

All membership applications (publishers and composers) are subject to approval by the FILSCAP
membership committee.

All members must constantly update FILSCAP on their works and should also try to actively monitor
their own works and advise FILSCAP about performances of their works.
