Complete the “Patient Screening” exercises (#s 1-5) located on page 19 in your Workbook. You
will find a template for this assignment located in the Doc Sharing section of the course.

Your Workbook Assignment is due by the end of Unit 1.

Submitting Your Work

Put your responses in a Word 97-2003 document. Save it in a location and with the proper
naming convention: username-HS200-section-Unit Assignment.doc (username is your Kaplan
username, section is your course section, X is your Unit number). When you are ready to submit
it, go to the dropbox and complete the steps below:

1. Click the link that says "Submit an Assignment".


2. In the "Submit to Basket" menu, select Unit X: Workbook Assignment
3. In the "Comments" field, make sure to add at least the title of your paper.
4. Click the "Add Attachments" button.
5. Follow the steps listed to attach your Word document.
To view your graded work, come back to the Dropbox or go to the Gradebook after your
instructor has evaluated it.
6. Make sure that you save a copy of your submitted project.

About this project


The goal of the Final Project is to apply the knowledge you have acquired in this course to a real-
life case scenario. The project is designed to incorporate the course outcomes of HS200.

Your Final Project will be due by the end of Unit 9.

Instructions for completion:


Select one Case Study from the list below.

Case Study #1: Diabetes


Hannah is a 10-year-old girl who has recently been diagnosed with Type 1 Diabetes Mellitus.
She is a 4th grade student at Hendricks Elementary School. Prior to her diagnosis, Hannah was
very involved in sports and played on the girl’s volleyball team. Her mother is concerned about
how the diagnosis will affect Hannah.

Case Study #2: Coronary Artery Disease (CAD)


Eric is a 47-year-old male who has just been diagnosed with Coronary Artery Disease. He is a
construction worker and spends a great deal of time away from his wife and three children. Eric
smokes approximately 1 ½ packs of cigarettes a day and enjoys drinking a 6-pack after a long
day at work.

Case Study #3: Hypertension (HTN)


Vicki is a 42-year-old African American woman who was diagnosed with Hypertension a month
ago. She has been married to her high school sweetheart for the past 20 years. She is self-
employed and runs a successful insurance agency. Her work requires frequent travel and Vicki
often has to eat at fast food restaurants for most of her meals.

Case Study #4: Pancreatic Cancer


Andre is a 69-year-old man who was just diagnosed with pancreatic cancer. He owns a chain of
Italian restaurants and is well known in the community. He is married and has a large family
including children, grandchildren and great-grandchildren. Every holiday all of his family is
treated to a home cooked meal that Andre prepares himself. This year he is concerned that he
may not be able to cook for an upcoming holiday. This depresses Andre and he feels depressed.

Case Study #5: Asthma


Tammy is a 17-year-old girl who was just diagnosed with asthma. She is a cheerleader for her
high school and is very busy with social events throughout the year. Next year Tammy will
attend View University on a cheerleading scholarship. She is very excited, but is a little nervous
about living away from home. Lately she has been having trouble controlling her asthma.

Case Study #6: Cerebral Vascular Accident (CVA)


Jan is a 72-year-old woman who recently suffered a Cerebral Vascular Accident. She suffered
damage to the right side of her body and is having difficulty speaking. She is widowed and has
no children. She lives by herself in a 2nd floor apartment. Jan is worried about how she will
care for her 3 cats, Daisy, Tempo, and Ms. Kitty.

For your Case Study Final Project, you need to include ALL of the following information:
1. Discuss the patient’s diagnosis. Include a definition of the actual disease or
condition.
2. Identify the factors which could have caused or lead to the particular disease or
condition.
3. Describe the signs and symptoms which are associated with the disease or condition.
4. Discuss the diagnostic testing that is usually performed in order to formally
diagnose the particular disease or condition.
5. Identify the appropriate treatment (including therapies, medications, etc) which the
patient may be prescribed for his/her particular diagnosis.
6. Discuss potential barriers to therapy which the patient may experience due to their
unique situation.
7. Discuss alternative treatments which may also benefit the patient.
8. Describe the typical prognosis for a patient with the disease or condition.
9. Identify patient teaching which would benefit the patient in your case study.
