STRATEGIC PLANNING for the NURSING SERVICE, ORGANIZATIONAL POLICIES & STANDARD PROCEDURES
I. I N T R O D U C T I O N Planning is defined as the first function of the management process which permeates into the other functions it serves as an anchor where the other functions of management such as organizing, directing, and controlling are dependent (Venzon, 2006). Importance of Planning: (Douglass, 1986) Leads to achievement of goals and objectives Gives meaning to work Provides for effective use of available resources and facilities Cost effective Characteristics of a good Plan: Precise Evidence-based Set on priorities Flexible Logical Practical Organization-wide Scope of Planning Top management o Composed of nursing directors, chief nurses and their assistants o Set the over-all goals and policies of an organization; their responsibility covers over-all management of the organizations nursing service Middle management o Composed of nursing supervisors o Direct the activities to actually implement the broad operating policies; formulation of policies, etc. for intermediate level for on-going projects and activities is done in coordination with top mgt. and those in the lower level Lower level (first-level) management o Composed of head nurses or senior nurses (+ charge nurses) o Do the daily and weekly plans for administration of direct patient care Types of Planning Operational planning o A detailed work plan for a coming year. o Focus: Sustaining the course of action and ensuring the employees ability to perform the designated tasks. Strategic planning o Carried out 3-5 years. (more than 1 year) o Systematic process that emphasizes assessment of the environment both internally and externally. o II. STRATEGIC PLANNING PROCESS 1. Assessment of the Environment It is the first step of any planning process. The managers must assess the environment: External- includes the competition for services in the community, take a look at the market and identifying customers, economic factors, political, educational, social and technological factors. Internal- Patient care standards, financial resources, human resources, research capabilities and staff development.
2. The Planning Hierarchy: Elements (Marquis, 2000)
MISSION STATEMENT Identify why the organization exists. They encapsulate the overriding purpose of the organization. It should be generally simple so that it can easily be identified and remember. Must be concise. NOTE: Who the target clients are and what services will be provided. It addresses its position regarding ethics, principles, and standards of practice.
VISION STATEMENT Sees, Envisions; identify the future of the organization, and accompanies the mission statement. It is an imagination/ dream. Examples: DOH: Health is a basic Human right. A continuous set of services must be provided to assure the enjoyment of this right especially the poor.
PHILOSPHY It describes the vision. It is the statement of Beliefs and values that direct ones life or ones practice. Example: from Quirino Memorial Medical Center The medical Center is guided by the ff beliefs: 1. The hospital is committed to assume a vital role in health promotion, disease prevention, curative, rehab and primary healthcare in partnership with public health counterparts, the clients, families, and communities.
. GOAL SETTING Aims; broadly stated; defines the ends. Be departmental, division, individual level; can be Short term or Long term goal. 2 types: Operational Goals- are the statements that indicate future directions in unit/ departmental level. It is more specific statements of the organizations vision. Example: Medicine Unit: a.) Increase nursing visibility within the medical center and the community. b.) Assure cost containment at the unit level. Strategic goals- are institutional goals often written by executives or upper level administrative members who oversee the organizations activities & are able to conduct environmental assessment. Example: 1.) Optimize the resources of the medical center. 2.) Emphasize clinical excellence.
. OBJECTIVES Specific than your goals, task Oriented, and done by individual/ Unit. They are stated in terms of results to be achieved and more concrete. Example: (from QMMC) Mission Philosophy Goals Objectives Policies Procedures Rules and regulations Example: The mission of DOH, in partnership with the people to ensure equity, quality and access to health care by:
>> making services available >> arousing community awareness >> mobilizing resources >> promoting means to better health
General Objectives: to provide the best possible health care services to its clients in a high quality setting conducive to attaining its vision as a center of wellness and a standard medical center. Specific Objectives: 1.) to provide a dynamic and innovative setting for clinical experiences of post graduate trainees, medical, nursing, and allied health affiliated students, and for continuing staff development, training and research.
Nursing Service part of the total health organization which aims at satisfying the nursing need of the patient, community. The nurse works with members of allied disciples such as dietetics, medical and social services in planning comprehensive program of the patient and patient care in the hospital. According to World Health Organization part of the total health organization which satisfy nursing objectives to provide prevention of disease and promotion of health.
Vision and Mission of Nursing Service: The mission statement tells the reason for existence of nursing service as defined by the nursing staff and in relation to service being provided in community of clients.
Philosophy of Nursing Service and Nursing Service Administration: Sets out values, concepts and beliefs that pertain to nursing administration and nursing practice within an institution or organization. They are abstract and contain value statement about as professional as it obtains to competence on nursing workers and about the setting in human being as clients, patients and workers Example: from QMMC: The Nursing division strongly believes that: 1. Its philosophy and objectives are congruent to the vision, mission, philosophy and objectives of QMMC. 2. the most important asset of an institution is its staff. To provide the best quality nursing care
Objectives of Nursing Service: Objectives are developed in collaboration with nursing personnel who will assist them. The major areas included in objectives are use of all resources, social responsibility, staffing, and requisition of supplies, planning education program, innovation, marketing and personnel preferences. Example: from QMMC: The Specific objectives of the Nursing service of the QMMC: (and there comes the objectives setting for a specific nursing unit).
III. NURSING STANDARDS
Standards are desirable sets of conditions and performance that ensures quality nursing service. Provide professionally desirable norms against which the departments performance can be measured. Although some institutions develop their own standards of Nursing Practice, the Standards of Nursing Practice and Nursing Service Admin formulated by a joint of committee of the ANSAP and PNA which was first printed in 1981, is a good reference.
Who sets the Standards: 1. Professional Organization (PNA) 2. Licensing Body (PRC) 3. Clinical Practice guidelines (OHNAP, ANSAP, ORNAP)
IV. NURSING POLICY Guidelines; It is a governing plan to achieve our goals and objectives; and are boundaries/ guidelines for decision making and behavior within which personnel can act. Three General Areas In Nursing That Require Policy Formulation:
Nursing Service Policy Manual is an example of a departmental polices, reflecting the policy of the hospital. Examples: 1. Accidents: Care, Reporting, Precautions to prevent occurrence. 2. Admissions: Receiving, Consent, Notifying Doctor, Care of Patients 3. Consent: Informed consent taken by/from whom,& shared decision-making. 4. Death: Notification, Care and Identification, Belongings, Certificate 5. Doctors Order: Written, Verbal, Telephone 6. Nursing Care: Administration and Prep of Meds, Charting, Safety Devices
V. NURSING PROCEDURES
Step by step activities. Procedures are specific directions for implementing specific policies. They are plans that establish customary or acceptable ways of accomplishing a specific task and delineate a sequence of steps of required action. Nursing Procedure manuals should be available in each unit to familiarize nurses with the common nursing procedures utilized in the unit. Format of the manual includes: definition, purpose, materials needed, proper requisitioning of supplies, proper disposal of used equipments, legal implications, expected responsibilities of the nurse and the patient, and proper documentation. Examples of content: Bed making, Insertion of Foley Catheter, Administration of Blood, I and O, Oral Hygiene, Administration of Medicines, Admission and discharging a patient.
