A. POINTS TO REMEMBER
1. The Firm Policy is to comply with the E-Filing Requirement by submitting the PDF copies burned on a CD and not by e-mailing the PDF copies in order to save on our bandwidth. Also, this is in consideration of our current e-mail capacity to send 30MB data (attachment).
2. By 2 October 2013, compliance is MANDATORY.
3. There should be no discrepancy between the ORIGINAL COURT PLEADING WITH AFFIDAVIT OF SERVICE, ANNEXES and VERIFIED DECLARATION (i.e., hard copy) and the individually saved SOFT COPY/IES (i.e., pdf/scanned files).
4. PDF copies may be viewed and printed. Always remember to restrict access to the PDF copies by setting a password to prevent alterations.
B. VERIFIED DECLARATION
1. Prepare a VERIFIED DECLARATION. Follow the template below.

2. The VERIFIED DECLARATION should be NOTARIZED.
3. Attach the ORIGINAL of the VERIFIED DECLARATION to the CD later on.

C. SCAN, SAVE, BURN, REVIEW, LABEL, ATTACH THE ORIGINAL VERIFIED DECLARATION, FASTEN, AND SEAL
1. Scan SEPARATELY in PDF FORMAT the following:
a. The ORIGINAL COURT PLEADING WITH AFFIDAVIT OF SERVICE (with attached Registry Receipt/s if served by registered mail and/or Receiving Stamp/Signatures if served by personal service; AND b. Each ANNEX should be scanned SEPARATELY

2. Save as SEPARATE PDF FILES (and save in a folder in the hard drive of the PC) and use the appropriate FILE NAMES: 1
a. The ORIGINAL COURT PLEADING WITH AFFIDAVIT OF SERVICE AND VERIFIED DECLARATION - TITLE OF THE PLEADING AND DATE OF THE PLEADING (e.g., PETITION FOR REVIEW 1 JULY 2013) b. ANNEX ANNEX LABEL (e.g., ANNEX A)
3. Secure DOCUMENT PROPERTIES of the PDF FILES and SAVE changes. Go to FILE Tab, click on PROPERTIES, go to:
a. DESCRIPTION (1) TITLE Type TITLE of the PDF FILE (2) AUTHOR Type ACCRALAW (3) Click OK
b. SECURITY (1) SECURITY METHOD Click on PASSWORD SECURITY from drop-down list, Click OK (2) PASSWORD SECURITY SETTINGS (a) Click on/Enable RESTRICT EDITING AND PRINTING OF THE DOCUMENT. (Note: Box must have ) PRINTING ALLOWED Click on HIGH RESOLUTION from the drop-down list. CHANGES ALLOWED Click NONE from the drop- down list. (b) Disable ENABLE COPYING OF TEXT, IMAGES AND OTHER CONTENT. (Note: Box must be empty ) (c) Disable ENABLE TEXT ACCESS FOR SCREEN READER DEVICES FOR THE VISUALLY IMPAIRED. (Note: Box must be empty ) (3) CHANGE PERMISSIONS PASSWORD Type ACCRALAW2272* (4) Click OK (5) CONFIRM PERMISSION PASSWORD Type ACCRALAW2272*
c. SAVE Always remember to SAVE after making the changes to DOCUMENT PROPERTIES.

4. Burn/copy the SEPARATE PDF FILES on to a CD/CDs. REVIEW/DOUBLE CHECK that the PDF copies saved on to the CD/CDs are complete and correct.
For example, a CD with PDF FILES of a Petition for Review and Annexes A C, should contain the following separate PDF FILES with the following file names: Petition for Review 1 July 2013, Annex A, Annex B, Annex C, Annex D, Annex E.
5. Label the CD/CDs.
a. For the LABEL, indicate CASE TITLE, CASE NUMBER, COURT, TITLE AND DATE OF PLEADING (Use CD Marker/Pen and write legibly and use of non-CD specific sticker paper for labeling the CD is discouraged) (e.g. Bautista v. Bautista, G.R. No. 12345, Supreme Court, First Division, Petition for Review 1 July 2013)
b. If more than one CD is used, label the CDs as 1 of 2, 2 of 2, and so on.
6. Place CD in a plastic CD sleeve then place inside a brown envelope.
7. Label the brown envelope and use the template below.

8. Attach/staple the ORIGINAL VERIFIED DECLARATION to the brown envelope containing the CD.

9. Fasten the brown envelope (containing the CD) and place on top of first page of the pleading.
If the pleading is not thick enough to be fastened, and pleading could be stapled together, then staple the brown envelope to the pleading.
10. Seal the brown envelope. Ensure that the CD will not fall off during handling.
D. REVISIONS TO TEMPLATE EXPLANATION FOR FILING/SERVING BY REGISTERED MAIL
The fact that a CD is being filed together with the hard copy should be indicated in the Explanation (if filed by registered mail).
1. If served on the other parties/lower courts by personal service and filed by registered mail
In compliance with Section 11, Rule 13 of the Rules of Court, this [TITLE OF PLEADING] with the scanned copy stored in a compact disc will be filed with this Honorable Court by registered mail due to [REASONS].
2. If served on the other parties/lower courts and filed by registered mail
