Required Materials:
1. iHealth: An Interactive Framework, 2nd edition (older edition of text is not acceptable).
Instructions for purchase are found later in this document. Read carefully.
2. DAILY access to a home computer with consistent, reliable internet service or to the GCC
computer lab (SG 129);
3. Knowledge of computers including: e-mail, internet operations, & word processing software;
4. Willingness to manipulate Moodle software and multiple other pieces of internet software.


Course Description:
Health 104 covers current issues in health and their effect upon the quality of daily living and
longevity. The course emphasizes the holistic approach to health and wellness and explores the
most recent concepts in nutrition, disease processes, disease prevention, emotional, and social
health aspects.


Class Conduct/Honesty Policy:
You are college students. You are expected to behave as professionals and to treat this course as
you would a job with non-negotiable work deadlines. Any form of cheating or plagiarism is
forbidden by GCC policy. Even the slightest hint of either will be reported to the Vice President of
the College. Any attempt to negotiate or argue with the instructor regarding any syllabus policy
may result in a drop. In the event of suspicion of academic dishonesty, the instructor reserves the
right to mandate an in-person meeting to discuss the work. Failure to attend will result in a zero
on the assignment/quiz and potentially a follow-up with the VP of Instruction.

Attendance Guidelines:
Before continuing with this syllabus, please read the GCC Attendance and Drop Policy for
online courses found here:

The information that follows is the attendance policy for THIS online course.
This course is self-directed. A book, video clips (some lengthy), audio clips and other online tools
ensure comprehensive learning. To pass this self-directed, 3-unit online course, plan to spend a
minimum of 6 hours on the work weekly. In a sixteen-week semester, according to GCC
policy, a 3-unit course requires three hours of classroom time and 3 or more hours of homework
time. For the purpose of this online course, classroom time consists of the following obligations:

1. logging into Moodle and reading all forums and email DAILY (this will be tracked)
2. contributing to group work at least once weekly per the assignment guidelines
3. contributing to Moodle forums periodically
4. submitting assessments weekly

There are additional methods by which you may indicate participation, but those on the list above
are MANDATORY. Failure at any point in the term to meet any combination of 3 or more of the
above obligations will result in a single warning by email and, if performance issues persist, a DROP
without opportunity for reinstatement.

Example: chapter 1 quiz missed + chapter 3 group assignment missed + failure to log into moodle
daily in week 6 = DROP

If this policy is not agreeable to you or not manageable for your personal time
schedule, you should not take this course. There are no exceptions to this policy.


Course Objectives:
Upon successful completion of the required coursework, the student will be able to:
Recognize and analyze signs and symptoms of disease and discuss these conditions using
basic medical vocabulary
Interpret and formulate an educated opinion about the reliability of multi-media delivered
health information
Explain the relationship between lifestyle choices and longevity, and will use this information
to implement and promote healthy living

Grading Policy:

Online Quizzes: 220 points
There is a required online quiz for each of the 11 chapters of the text. Quizzes will consist of 10
single and multiple-response questions, as well as matching questions, which must be completed in
20 minutes. Quizzes are closed-book, closed note. Total points possible for each quiz is 20. One
quiz is required per chapter and quizzes are always due Tuesdays by 5pm. If performance
is poor on a quiz, it may be repeated, as long as the quiz is still available. There is a 30-minute
forced time restriction between quiz attempts during which the student should review the material.
Two missed quizzes (including non-consecutive missed quizzes) constitutes poor course participation
for which students may be dropped at ANY time during the term.

Group Participation Assignment: 55 points
There is a worksheet to accompany each text book chapter. This worksheet is located in server-
based software and is accessible to groups of about 20 students. Collectively, the group completes
this worksheet according to its specific instructions. Failure to contribute to the worksheets each
week may result in DROP for non-participation. Students are advised to complete the participation
assignment early each week, as some worksheets are short and questions are limited. Earning
participation credit on this assignment is first-come, first-served, meaning no credit may be available
for students waiting until the last minute to contribute. Regardless of the reason, failure to
contribute is grounds for drop due to non-participation.

Project: 100 points
After completion of the chapters in the text, there will be a project assigned. Project details will be
provided at the time of the assignment.

Essay: 100 points
An essay replaces a final examination in this course. This essay is formally assigned as of
completion of other course work.


Final Course Grade Calculation:
The course grade will be determined by dividing the number of points the student earns by 475. No
incompletes will be granted and grades will not be curved. Please avoid requesting a better grade in
the event that you fail to earn the grade you want, as this would engage your instructor in academic
dishonesty, something punishable by suspension from the College.
90-100 = A 80-89 = B 70-79 = C 60-69 = D <60 = F


Students With Disabilities:
All students with disabilities requiring accommodations are responsible for making arrangements in a
timely manner through the Center for Students with disabilities. Any accommodations to
assessments will require students to take the assessments at the GCC Center for Disabled Students.

Online Etiquette in Health Education

The online classroom is very different (at least in this course) from social media.
Professional classroom etiquette and language is expected in all communications for
this course.
1. Your instructors name is Ms. Erfurt and she must be addressed as such.
2. You are expected to use proper English grammar (no foul language) and type in
complete sentences in all communications.
3. Texting language (such as cn u c me? or LOL) is NOT an acceptable form of
communication in this college course.
4. The instructor reserves the right to DROP students who fail to meet the etiquette
expectations stated here.

A check-list containing everything required of students is provided at Moodle (after
orientation is successfully completed). It should be downloaded and used
throughout the term to assure that no work is missed

Lecture and Prezi note-taking guides are documents that help you make quality notes
on the online video elements
Note-taking guides are NOT submitted for points
It is recommended that note-taking guides be completed while watching the online
elements, then used to prepare for ALL QUIZZES

DISCUSSION
Answers to text review worksheets and clarifications related to online elements and
note-taking guides will not be provided by the instructor, but may be discussed
online using the moodle forum links called DISCUSSION
It is not acceptable to directly ask other students for answers to assignments.
Rather, students must first post their own response, then make a request for
additional information.
The instructor will oversee this discussion link for accuracy, and will post clarifications
if necessary

Text Book Purchase Instructions

This page contains text book purchase information. It is recommended that you read this
entire syllabus and complete the orientation full before buying the text.

You need to purchase the book titled iHealth, An Interactive Framework. You ONLY NEED
THE BOOK. You have many buying choices. The GCC bookstore is NOT one of them. This
book (in print) costs $125. You are better off using an e-book. There are many web sites
renting/selling e-textbooks cheaply. Try www.coursesmart.com as a start. You must buy
or rent chapters 1-11, as thats what we will cover this term. You need not buy the
Connect software access that accompanies the text.





The text book can be purchased in print directly from the publisher at this web site. Make
sure you purchase the second edition (black cover).

The hard copy of this book is not sold through the GCC bookstore due to expense.
However, if you qualify financial aid that pays for books, you may request that the
bookstore order the book for you so you can use financial aid to purchase it.

Additionally, there is a copy of the text book (in print) available for you to use in the GCC
library at the reserve desk if you prefer NOT to use an e-book. Take your student ID card
and submit it to the front desk in exchange for use of the book at the library for 2 hours.
The book may not be removed from the library.

Quiz Guidelines

Because significant issues with dishonesty in quiz-taking exist at GCC, the guidelines for online quizzes are
very restrictive. If you believe you will struggle with completing the quizzes per these guidelines, you should
not take this online course.

There will be multiple choice questions and matching questions on each chapter quiz. Multiple choice
questions may have a single answer or multiple responses. Collectively, the responses are
worth 20 points. The questions will not have obvious answers. You will need to integrate information
from the chapter in order to perform well on the quizzes. Completing the group worksheets and note-
taking guides and drilling yourself with the flashcards will help you learn the necessary details.

Each quiz covers only one chapters worth of new information from the text and online elements, but
information from past chapters will also be intergrated. All chapter quizzes are available as of
completion of online orientation. Quizzes are due Tuesdays by 5pm per the course check-list.

Two missed quizzes (including non-consecutively missed quizzes) constitutes poor course participation
for which students may be dropped with or without warning at any time during the term.

The quizzes are CLOSED BOOK and CLOSED NOTES. No dictionaries or other tools of any kind
are allowed during the quizzes. You may not speak with, text message, communicate with,
or work in any other way with other students or non-students during your quiz. The
instructor reserves the right to mandate an office hour with students in the event that there is suspicion
or evidence of academic dishonesty. Failure to respond or attend will result in a substantial point
penalty.

Once you begin your quiz, you will be under a 20 minute time constraint. Questions can only be seen
once. Once your response is saved (or unsaved), questions may not be revisited. Any unanswered
questions or unsaved answers will be marked wrong.

Quizzes may be taken in the computer lab or anywhere else. Students who opt to take quizzes
outside of a GCC computer lab assume all responsibility for questions missed due to
computer/server problems. If a technology error occurs during a quiz, notify the instructor
immediately. An inquiry will be made by the instructor to GCC tech support. If GCC tech support
indicates a problem with GCC technology, AND the student request for a quiz review occurs within the
window of quiz availability, the quiz will be reset and must be completed within the original window of
availability. If GCCs tech support does not diagnose an internal GCC server failure, any missed or
unfinished questions will be marked wrong. This policy is not negotiable. Students attempting to
negotiate the policy will be encouraged to drop the course and take it in a traditional classroom setting
void of technology risks.

Quizzes may be repeated in the event of a poor score, provided the quiz is still available.
There is a 30-minute forced time restriction between attempts, which serves as review time for the
student. In cases where quizzes are repeated, the scores from the 2 quizzes will be averaged. Correct
answers will be available for viewing online at the end of the period of availability for each quiz.

Scores will be available online at the MyGrades tool shortly after the quiz availability ends. For questions
that contain multiple correct responses, points will be granted for all corretly marked responses. Points
will be lost if incorrect answers are marked as correct or if correct answers have not been marked as
correct.

It is recommended that quizzes be taken between the hours of 7am and 11pm, as the GCC server may
become less reliable at night. Any problems with technical issues should be addressed to the GCC
helpdesk at ehlp@glendale.edu or 240-1000 x 3457. Please cc your instructor on any mails you send.

Education 104 this fall term 2014.

I understand that if I do not complete my online orientation in full by the deadline assigned to me
by the syllabus, I will be dropped from this course and will have to wait-list myself if I wish to
rejoin.

I have read the GCC college-wide Attendance and Drop Policy and the specific attendance policies
for this course. I understand and agree to uphold my attendance obligations in this course. I
understand that if I am dropped, there will be no opportunity for reinstatement this term.

I understand that this course involves earning points through weekly online quizzes, a weekly
online group assignment, a project, and a final course essay. My earned points, collectively,
constitute my final course grade.

I understand that my grade is calculated mathematically based on my quiz scores, exam score, and
project scores, and that my total points will be divided by 475 to determine my course grade.

I understand that though there MAY be some, there will be no guarantee of an opportunity for
extra credit, that no earned grades will be dropped, and that final course grades will not be curved.

I understand that I will be using technology that has the potential to fail or crash from time to time.
I have been advised that exceptions will not be made with regard to due dates for course work as a
result of technology issues that are not directly related to GCC.

I understand that because this course is self-paced no extensions or exceptions will be made for
ANY reason as far as due dates go. I agree not to request any per this policy.

I understand that if I request a final grade in this course OTHER than that which I earned at the
end of this term that I will be attempting to engage my instructor in academic dishonesty,
something punishable by suspension from the College. I agree not to do this.

I have read the cover letter and syllabus detailing the Health Education 104 requirements in full
and agree to it and to the terms of this contract.



You do not need to submit this form with a signature. Rather, you will encounter a
question during online orientation (more about this on subsequent pages of this doc)
asking whether you agree to these terms. You type your name there if you agree. If
you do not electronically sign your quiz by typing your name when prompted or if you
fail your quiz after 2 attempts, you will be dropped from this course.

ONLINE ORIENTATION

Due to the weekend holiday, you have the option to complete an online orientation this
term for this course, at which time you will be excused from attending the in-class
orientation.
THE ONLINE ORIENTATION DEADLINE IS FRIDAY 8/29 (5 PM)
If you have not completed orientnation by that time, you will need to attend in-prerson
orientation held in SN101 on Saturday, August 30th at 11am. Failure to do so will
result in DROP.

BEFORE DOING THIS CHECK-IN, MAKE SURE YOU HAVE READ THE SYLLABUS IN FULL
AND UNDERSTAND YOUR OBLIGATIONS IN THIS SELF-DIRECTED ONLINE COURSE.
THE FOLLOWING STEPS ARE DESIGNED TO HELP YOU GET YOUR COMPUTER SET UP TO
OPERATE EFFICIENTLY IN THIS COURSE AND TO ASSURE THAT YOU UNDERSTAND THE
PROCESS OF GETTING YOUR QUESTIONS ANSWERED RAPIDLY.

1. Health Ed site: Open a browser window and go to http://www.healthliteracymatters.com.
This is our home base which from now on will be called the Health Ed site. Everything you
need can be accessed from here. Please bookmark this site, then watch the Health Ed tutorial.
2. Moodle: At the Health Ed site, click Moodle, found on the green bar. Note that you can link to
the moodle login page or to help resources for moodle. We will use moodle for forum
discussion, messaging, and quizzes and downloading of individual assignments.
Log in to Moodle: Familiarize yourself with the format of Moodle. Not much is available
to you yet, but as of the conclusion of this orientation, more will appear. If you are not
already comfortable using Moodle, it is suggested that you take an instructional workshop
on the use of Moodle software offered by the GCC IT department. Find the workshop
schedule by calling the helpdesk at x3457. Opting not to take a workshop, then failing to
properly use the software may result in a poor course grade. There are no retakes of
quizzes resulting from students lack of understanding of the Moodle software. This course
will not teach you to use Moodle or any other software.
Asking General (Public) Questions*: Once you start working on your course material,
if you have a question on general course business, here is the procedure:
o REVIEW in detail this SYLLABUS to see if the answer is hereit probably is.
o READ the existing postings in the FORUM link called GENERAL COURSE
BUSINESS DISCUSSION to see if the question has already been asked and
answered
o If the question remains unanswered and it pertains to a single chapter, read the
DISCUSSION link pertinent to that chapter.
o If you STILL cannot find an answer, post a new question to the GENERAL
COURSE BUSINESS DISCUSSION LINK for EVERYONE to view. This link helps
serve as our general classroom situation. Other students may benefit from your
question or may help you answer your question here as though you were asking it
in an in-person class session.* If you see a question that you are able to answer,
please go ahead and answer it! Refrain from asking repeated questions, as it
wastes everyones time. Repeated questions will not be answered, as it
is your responsibility to keep up with posts. Find the GENERAL COURSE
BUSINESS LINK in Moodle now, so you know where it is for future reference.
Private Questions*: Please message the instructor directly with any personal issues or
items (including grade issues) not appropriate for viewing by the class. Messaging link is

along the right margin in Moodle. Locate it now for future reference. The instructor
will NOT REPLY to general questions sent by message.
Reminders: Announcement reminders will be posted daily at Moodle in the
Announcement box. You are required to check your announcements and
discussion links every day for important changes, updates, or other items of interest
that may be posted there during the term. Your attendance will be tracked. Failure to
login daily may result in drop, as it is an indication of non-participation in this
course. There is no need to reply to updates unless a reply is specifically requested.
7. Video Lectures: Go to the Health Ed website. At the left margin, click on Video Lectures.
This page houses links to your video lectures. Video lectures explain and elaborate on what
you read from the text.
o Video Lecture check: Click on several chapter lectures to make sure they operate
correctly. There will be audio and video components.
8. Prezi access: At the left margin of the Health Ed site, click on Prezi Links, then click on
Orientation Prezi. You will be prompted to set up a free account. Once the account is set
up, be sure to watch the orientation prezi to familiarize yourself with how prezis work.
9. Quizlet (flashcards): Return to the Health Ed site, then click on the Quizlet link to make
your free quizlet account. Once logged into the quizlet Health Education site, bookmark it
into your browser so you can easily locate the flashcards repeatedly.
10. Google homework folder: You will be submitting assignments this term through software
called google drive. In order to do this, you must set up a folder using specific guidelines.
From the Health Ed site, click on Tutorials in the left margin. Then, view the tutorial called
"How to set up google drive and submit homework. Follow these instructions in full to set up
your homework folder.
11. Review any additional links you havent already visited at the Health Ed website, as they will
help clarify how things work.
12. Syllabus quiz: It is now time to take the Syllabus quiz (Moodle > Syllabus Quiz). Be sure
you have read and understood everything contained in this document to prepare for the
Syllabus quiz. Also keep this document handy for use during the quiz (it is open notes). You
must complete this quiz and view your score in order to access the course materials. If you
earn a score below 70%, you must repeat the quiz. If you fail twice, you will be
DROPPED from the course. This course requires attention to details. If you cannot pass a
quiz over the syllabus, you are unlikely to succeed at the course work. The points earned on
the syllabus quiz will NOT count toward your final course grade. This quiz may have single
response answers or multiple response answers (like chapter quizzes in the course)
13. Online text book access: AFTER you have passed the syllabus quiz, return to page 6 of
this syllabus and follow the instructions for purchasing your text book. There is also text-book
access from the Health Ed web site under Textbook.
14. Excusal from in-class orientation: If you feel you adequately understand how this course
works, and you have completed all of the above steps, send a moodle message to Ms. Erfurt
requesting an excusal from the in-class orientation. If you fail to request this excusal
and you miss in-class orientation, you will be DROPPED.

Start the course by going to the Moodle homepage and locating the document called
Health 104 Check List. Download and save or print it. Open it and you will see a
to-do list for each chapter. If you are on track, you will have already completed all
of the items listed under Pre-course Instructions and you can move directly to
Chapter 1.
