COSHH Risk Assessment April 2009

A COSHH risk assessment is required for work with hazardous substances including source materials, products, known
intermediates and by-products. The form should be completed electronically and approved and signed by the principal investigator
or responsible person. Carcinogens and users of carcinogens must both be registered using the Carcinogen Registration forms on
the Chemical Safety section of the Safety Office website. Guidance on completing this form is provided in the COSHH Risk
Assessment section of the Safety Office website.
