in accordance with law hereby depose and state:

That I am the true and lawful owner of a Mercedez Benz X-5 covered by CR No. 123456
and OR No. 654321 which is more particularly described as follows:

Engine No: 00000011 Chassis No.: 00000011
Make : Mercedez Be Year Model: 2012

That sometime in November 15, 2011 while on my way to the gym at Makati and
carrying a bag containing the CR and OR of the above described vehicle, the bag was
stolen by robbers;

That said bag containing the CR and OR of the said vehicle was lost and never recovered;


That I am executing this affidavit to attest to the truth of the foregoing and to request from
the Land Transportation Office of Manila City, a copy of said Certificate of Registration.
