P PRINCIPLES RINCIPLES FOR FOR S SAFE AFE O OPERATIONS PERATIONS
1. Hazards must be analyzed, identified and controlled.
Section 1208: Explosive Hazards i. Before striking an arc or lighting a torch in a confined space, the welder must be sure that the space is free of explosive atmospheres such as those caused by natural gas, coal dust, flammable vapors, etc. ii. Before starting to weld the tank or pipe container, the welder must make certain that it contains no explosive mixture or substance. iii. Combustible floors shall be kept wet or protected by fire-resistant shields. 2. Isolations of work hazards must be adequate and secure. Section 608: URD - General i. Before a URD transformer enclosure is opened, all unauthorized persons, including private citizens, shall be required to leave the work area, and remain clear of all the hazards involved in the work. ii. When underground equipment is being located, short sections of scrap cable could provide false indications of the actual position of permanent conductors; therefore, all scrap cable, regardless of length, is to be removed from the job site. 3. Personnel must be trained to do their jobs correctly and safely. Section 1411: Industrial Truck. Fork Lifts i. Industrial trucks shall be operated only by authorized persons who are qualified an trained in their use. ii. Before moving the equipment, the operator shall make sure that no person or objects are in the path of the vehicle. iii. Loads shall not be suspended or swung over other persons. No one should be allowed to stand or walk under elevated forks. iv. The operator shall always look in the direction of travel v. On inclines, all types of loaded lift trucks shall be driven with the load on the upgrade side of the driver whether ascending or descending. vi. All loads shall be securely fastened or safely positioned to prevent tipping or falling. vii. No one shall be allowed to ride the truck, fork lift, or other equipment - other than the operator, except when seats are provided for this purpose. viii. When an industrial truck is left unattended, the load engaging means shall be fully lowered, controls shall be neutralized, power shall be shut off and brakes set. Wheels shall be chocked when the truck is parked on an incline. ix. Equipment shall always be operated at a safe speed. x. Only attachments provided by or approved by the manufacturer may be used; all attachments shall be properly secured. Improvised methods shall not be used. 4. Facilities, equipments & parts shall meet all requirements. Section 123: Portable Electric Tools i. The non-current carrying metal parts of portable electric tools such as drills, saws and grinders shall be effectively grounded when connected to a power source unless: 1. The tool is an approved double-insulated type, or 2. The tool is connected to the power supply by means of an isolating transformer or other isolated power supply, such as a 24V DC system or GFI. ii. Electrical tools shall not be used where there is a hazard of flammable vapors, gases, or dusts. 5. Procedures must be written clear, approved and understood. Section 140: Radio Communications i. The Company has established radio procedures and priorities. All employees who transmit on our assigned frequency will know and follow these procedures. Section 141: Radio equipment 1. Only qualified employees shall work on the inside of transmitter cabinets. 2. When an employee is working in a transmitter cabinet, the power supply shall be shut off unless tuning or testing operations are required. 3. Before radio equipped cars or trucks are driven adjacent to or beneath energized equipment, such as outdoor substations, a check shall be made to insure that proper clearance will be maintained between the antenna and energized equipment. 4. Transmitters in trucks and cars shall not be used while the gasoline tank of the vehicle is filled. 6. Changes must be analyzed for system and parts risks. Section 609: Opening and Closing Circuits - URD i. Company switching procedures, including Hold Carding and tagging practices, shall be followed when sectionalizing URD Systems. Note: Refer to section 1700 Electric Line Clearance and Switching Procedures Section 1705: Switching Instructions and Procedure 1. Employee involved in switching shall read, and follow the Company switching procedures. Note: refer to Operating Procedure No. 1 7. Residual hazards must be monitored and controlled. Section 419: Loss of Ignition If ignition is lost when a unit is in operation and the furnace is hot, considerable hazard is present in relighting. i. All fuels must be shut-off immediately, including torches. ii. If the unit is operating on automatic combustion control, being careful not to increase air for about 5 minutes. It is then mandatory that the 5-minute purge be used before again lighting off. 8. Management shall control systems safety programs. Section 613: Excavations - URD When operating the underground system, all applicable rules for overhead distribution shall apply. In addition the following special rules shall be in effect and considered part of the overall safety program. See more on Underground Line and Equipment, pp. 40-41.
