a direct functional and structural connection between living bone and the surface of a load carrying
implant
1st : wound is filled with blood clot around implant
2nd : provisional CT established by granulation tissue replacing part of clot
3rd : 1 week later (rich vascular and mesenchymal cells)
4th : two weeks later, the formation of immature bone in mesenchymal tissues around titanium
surface.
5th : 6-14 weeks later mineralized bone primary and secondary osteons formed (Remodeling)
trabeculae to lamellae and marrow.
Classification Of Dental Implant
According to Design
1- Endosteal Impant : a. blade implant b. ramus frame implant c. root form implant
2- Subperiosteal Implant
3- Transosteal Implant
4- Intramucosal Implant
According to attachment mechanism
1- Fibro-integration
2- Osseo-integration
According to macroscopic body of implant
1- Cylindrical dental implant
2- Threaded dental implant
3- Plateau dental implant
4- Perforated dental implant
5- Solid dental implant
6- Vented dental implant
7- Hollow dental implant
According to implant surface
1- Smooth surface implant
2- Machined surface implant
3- Textured surface implant
4- Coated surface implant
According to implant material
1- Metallic implant
2- Ceramic or ceramic coated implant
3- Polymeric implant
4- Carbon implant
Peri-implantitis
inflammatory process around an implant characterized by inflammatory lesions in peri-implant
mucosa and loss of peri-implant bone.
mostly affecting the marginal part of peri-implant mucosa.
Radiographic bone loss, crater shaped loss, BoP , Suppuration.
Peri-implant Mucositis
reversible inflammatory process in the soft tissue surrounding a functioning implant.
assessed by BoP
CIST Protocol Charts
Mechanical debridement; CIST protocol A
BoP positive, suppuration negative , PPD 4 mm
Antiseptic therapy; CIST protocol A+B
BoP positive, suppuration negative , probing depth (45 mm)
Antibiotic therapy; CIST protocol ABC
BoP-positive , deep pockets (PPD 6mm) , (suppuration may or may not be present) , radiographic signs
of bone loss
Regenerative or resective therapy; CIST protocol ABCD
BoP negative , no suppuration , reduced probing depth
Bone Density assessment classification
D1 : quality 1 comprises homogeneous compact bone
D2 : quality 2, a thick layer of compact bone surrounding a core of dense trabecular bone
D3 : quality 3, a thin layer of cortical bone surrounding dense trabecular bone of favorable strength
D4 : quality 4, a thin layer of cortical bone surrounding a core of low-density trabecular bone
Components of implant restoration
1- Implant Body a. body ( screws, baskets, plateaus, balls, cylinders, diameters, lengths, prosthetic
attachment, surface condition)
b. Crest module (retain prosthetic component)
2- Prosthetic abutment (screw retention or cement retention)
3- Prosthesis
