To render diagnoses and guide therapy, pathologists iden
tify changes in the gross or rnicroscopic appearance (,rol
pDology) ofcells and tissDes, and biochmical altrations
in body fluids (such as blood and urine). Traditionally,
the discipline is dividd into general pathology and sys

Cells are active pa.ticipanrs in their environmenr, con-
stanrly adjusting their structure and function to accom
modate changing demands and extracellular stresses.
Cells tend to maintain rheir inrracellular milieu wirhin a
fairly narrow range of physiologic parameters; thar is,
rbey maintain notmal homeostasis. As cells encounrer
physiologic stresses or pathologic stimuli, they can
uodergo adapiation, achieving a new steady state and
p.esenins viability and function. The principal adaptive
the latter examines the parricular responses of specialized
organs. In this book we 6rst cover the broad principles
ofgeneral pathology and then progress ro specific diseasc
processes in individual organs.

The relationships between normal, adaprec, ano
reversibly and irreversibly injured cells are well illustrated
by the responses of the heart to different types of stress
(Fig. l-2). Myocardium subjected to persistenr

Most forms of pdtrolog,c hyperplasia are caused by
ercessive hormonal or growrh facor srimularion. For
exampl e. af t er a normal men, rrurl peri od rhere r. a

tightly regulated by stimulation through pituitary hor-
mones and ovarian esrrogen and by inhibition through
progesterone. However, if the balance between esrro-
gen and progesterone is disturbed, endornerial byper
plasia ensues, a common cause of abnormar mcnstruar
bleeding. Hyperplasia is also an imporrant response of
connective-tissue cells in wound healing, in which pro-
liferating fibroblasts and blood vessels aid in re-pair
(Chapter
3). In this process, grofth factors are p.o-
duced by white blood cells (leukocytes) responding to
the injury and by cells in the exrracellular matrix. Srim-
ulation by growth factors is also involved in the hyper-
plasia that is associated with cenain viral infecionsi
for exarnple, papillomaviruses cause skin warts and
mucosal lesions composed of masses of hyperplastic
epithelium. Here rhe growrh facors may be produced
by tbe virus or by infecred cells. It is important to note

Causes of atrophy include a decreased workload (e.g.,

The term narrosls was first used by morphologisrs to refer
to a series of changes that a.companl cell death, largely
resulting from rhe degradative action of enzymes on

enzymes responsible for digestion of the cell are derived
either from thc lysosomes of the dying cells rhemselves or

The cel l may have a more gl assy

because of the loss of glycogen particles. When
enzymes have digested the cytoplasmic organeiles, the
cytoplasm becomes vacuolated and appears moth
eaten. Dead cells may be replaced by l6rge, whorled
phospholipid masses, called myelin figuret that are
derived from damaged cellular membranes. They are
thought to result from dissociation of lipoproteins with
unmasking of phosphatide groups, promoting the
uptake and intercalation ofwater between the lamellar
stacks of membranes. These phospholipid precipitates
are then either phagocytosed by other cells or funher
degraded into fatty acids; c6lcification ofsuch fatty acid
residuas results in the generation of calcium soaps.
Thus, the dead cells may ultimately become calcilied.
