CONFIGURATION MANAGEMENT QUESTIONNAIRE
Administration and Analysis
Purpose:
To obtain insights into possible configuration management (CM) strengths and weanesses based
on input from plant personnel
Administration:
The CM !uestionnaire within this attachment should be pro"ided to the station as part of the
information re!uest letter# The plant should administer the !uestionnaire and return the
completed !uestionnaires ideally with the other information re!uested for the e"aluation# The
!uestionnaire results should be returned no later than three wees prior to the e"aluation#
Twenty to $% indi"iduals from each of the following plant departments should fill out the CM
!uestionnaire& engineering (design and plant support)' maintenance including planning and
scheduling' operations including operations support# The indi"iduals completing the
!uestionnaire should include indi"iduals ranging from managers to worers# The station should
pro"ide a minimum of (% responses' but no more than )%# A sample of si*ty to ninety responses
is ade!uate to identify general themes or trends#
Analysis:
+esults should be analy,ed for general themes or trends implying strengths or weanesses in the
different aspects of CM# Note tat results are opinions and pro!ide only possi"le CM temes
o# stren$t or %ea&nesses' Tese indi(ations usually need !alidation trou$ additional
data re!ie%s) o"ser!ations) and inter!ie%s#
-t can be helpful to tabulate the !uestionnaire results on an .*cel spreadsheet# This can help
identify general themes or trends# -/P0 1upport 1er"ices has pro"ided this tabulation in the
past#
Analysis results should be considered with other e"aluation related information to help identify
CM e"aluation focus areas as well as helping to identify those areas that appear not to need
further e"aluation#
Results:
-t is recommended that the !uestionnaire results be shared with the plant# 0ne techni!ue is to
share the results and analysis with the CM counterpart team during e"aluation preparation or
upon arri"al on site to start a dialog on the areas of interest# 2ou may as the counterpart team to
perform their own analysis and then compare results# The results can pro"ide a basis upon which
the e"aluators and counterparts can wor as a CM team during the e"aluation# Te

SITE INSTRUCTIONS:
The configuration management !uestionnaire consists of three different !uestionnaires' one for
the operations area' one for the engineering area' and one for the maintenance area#
.ngineering area personnel include design' system and other site support engineering such as
performance engineering' test engineering' pro9ect engineering etc#' depending on the site
organi,ation#
0perations area personnel include the operations department and operations support
organi,ations# 4or e*ample' some plants ha"e operations support organi,ations that support
operating procedure maintenance' scheduling or tagouts# Again' this depends on the specific site
organi,ation# The intent is to get operations:operator input on the !uestionnaire#
Maintenance area personnel include those in the maintenance organi,ation as well as in planning
and scheduling# Personnel in separate outage preparation organi,ations should be included in
this group#
The !uestionnaire should be filled out by 2% to $% indi"iduals' ranging from the management
le"el to worer le"el' in each of the areas for a total of (% to )% completed !uestionnaires#
-deally' All the completed !uestionnaires should be pro"ided to -/P0 at one time with the other
re!uested e"aluation information# -f this is not possible the completed !uestionnaires should be
returned to -/P0 no later than three wees prior to the e"aluation on site#
