Reported headaches, dizziness, nausea and bleeding noses. 1. Forthwith Order issued under section 25(1)(b) of the OHSA: The employer shall ensure that, eyewash fountains located at the E-coat line are maintained in good condition. At the time of the visit they had not been recently inspected (weekly) and were not in a sanitary condition. The order was complied with before the inspector left the workplace. 2. section 25(1)(b) of the OHSA: The employer shall ensure that, grounding cable on the flammable cabinet, located at the E-coat line is properly connected to the cabinet. During the visit the cable was found disconnected. The order was complied with before the inspector left the premise.
Y July 22, 2013 2 Event Other Injury A workers hand was lacerated working with sheet metal 1. Time based order issued under section 24 for guarding. This order applies to the use of a chain connected interlock switch at the access gate to cell # 6.
2. Stop work order under section 57 (6)(b) re: the above.
3. Time based order issued under section 24 for guarding. This order applies to the front parts access opening on cell # 6 on press line # 6.

Y Sept. 19, 2014 3 Fatality A worker in the Paint Line area was killed when bins containing parts fell on the worker. 1. Order issued under section 45(a) of the Industrial Reg. 851/90 requiring that material, articles or things shall be moved with all safeguards and precautions so as not to endanger the safety of any worker. This order was specific to the Paint Line area.
