Agreement of Contract Principles

Please place a check mark on each box upon your agreement with the contract.

1. ALT should submit all the required documents within the probationary period.

2. First payment will be made on 25th of following month. (ex : If you start
working from April, your first payment will be made on 25th of May.)

3. The company provides employees all the necessary documents to apply for
work visa. However, it is employees sole responsibility to apply for the visa
in person at an immigration office, and all miscellaneous charges;
stamp duty; transportation, will always be the employees sole responsibility.

4. Before the start of each employment year, the employee shall undergo a
medical check examination.

5. The employee will be required to obtain a telephone as soon as he/she is in
possession of an Alien registration card.

6. When the employee decides to resign from his/her services to the
company, a minimum one-month (30 calendar days) notice of resignation
must be given. If this length of notice is not given, the employee agrees
to have related damages (up to 150,000 yen) deducted from his/her salary.

7. Income tax will be deducted at source. The employee should be aware that
he/she is responsible for any shortfall in the tax assessment.

8. In the event of lateness or any case of absence from work, the
employee shall make all attempts to re-schedule those hours or days missed
within the month in question. Where re-scheduling those hours or days is not
an option for any reason, the employees salary shall be reduced as per the
amount stated on the contract.

9. When it becomes apparent that employee will be late or absent from work,
he/she is to inform the company/staff as soon as possible by phone.
NB: A text message is not sufficient. The employee should leave his/her voice
message on companys answering machine during non-contactable hours.

10. For a period of two or years after the termination of employment, the
employee is prevented from approaching or accepting any employment
position directly or indirectly with any Heart School Client to whom the
employee was introduced by the company during the tenure of their
employment with Heart School.

11. Occasionally, some costs must be viewed as Employees induction expenses,
and not the Companys. In some instances, mandatory training sessions
and/or introduction meetings will take place, and the transportation costs
to the necessary locations will not be reimbursed by the company.
The date of these meetings can be changed by Heart School and the employee
will adjust accordingly.

12. If using Company accommodation, I request for Heart Corporation, to
finalize the costs of staying in the Company apartment. I understand that
final pay may be withheld until such finalization is possible, in instances
such as an inability to vacate the apartment within the terms of the
contract, or other damages related to use f the apartment, and that final
pay will be adjusted according to any resulting costs.
