Thank you for participating in the Zooppa contest! We look forward to receiving your
submission. In order for your submission to be complete, we need some documents
from you:
(1) Signed Talent Release. See Page 2-3 below. All the talent included in your video
must sign this release form so that our client may review and use their images
and performance in the video submission. If we don't receive your signed
releases we will not be able to accept your submission as part of the contest.

(2) Signed Audio License. See page 4-6 below. This form provides us with
information for the music that is in your video. Please fill out the applicable form
based on whether the music is an original piece created by you or if you have
obtained a license to the music.
Please complete both forms and upload them into your profile. We thank you for your

ZOOPPA PERFORMANCE CONSENT AND RELEASE
This Performance Consent and Release (Release") is given by the undersigned actor,
model, extra, who is an individual residing at the address listed below (Talent) to
Zooppa.com Inc. (Zooppa) and its assignees. Talent acknowledges that the assignee
refers to the applicable brand and when applicable the brands client for each respective
contest.
The Talent irrevocably consents to the reproduction, public display, performance
or any other use of the Talents Performance, submitted as part of a Zooppa contest
entry, to be used anywhere in the world, on any media available now or devised later.
Talent recognizes that Performance includes the performance, Talents appearance,
likeness, voice, biographic information, name, and any other personal rights. Talent
acknowledges that Zooppa, or its assignees, may edit the Performance, incorporate it
into other productions and performances, add special effects, sound effects, music,
photographs or other media of any kind to the Performance and use it for publicity,
advertising and promotion of Zooppa or its assignees and, except for applicable equity
actors residuals (if any), Talent is not entitled to, and will not seek, additional
consideration, compensation, payment or proceeds in exchange for this consent and
release agreement.
Should this agreement and consent fail to operate to transfer to Zooppa, and its
assigns, the rights as it may need to reproduce, use and publicly display the
Performance in any manner it deems necessary, Talent agrees to assigns to Zooppa
and its assignee an exclusive right and royalty free license to use the Performance, and
the right to sub-license such rights, under any law worldwide, including any publicity,
privacy or copyright law, in perpetuity.
Talent releases Zooppa, its legal representatives and assignees, from all claims
and liability that Talent may have arising from these rights, the Performance or the
reproduction, use and display of the Performance by Zooppa and its assignees.

Consent and Release agreement and agree to be bound by its terms.
