Requirements for CHED Application for Student Financial Assistance Program (STUFAP) 2009

SCHOLARSHIP
1. Certification from the Principal - that belong to the TOP 10 of the 4th year high school
class;
2. Photocopy of ITR Certificate - Parents combined gross annual income of not more than
150,000.00 pesos;
3. Photocopy of NSQE/NCAE result - with average percentage of 90 and above;
4. Photocopy of Birth Certificate;
5. Not more than 25 years old at the time of application;
6. Certificate of Good Moral Character;
7. Certification from the Barangay – as to the total number of children in the family;
8. Form 138 - 3rd grading period with GWA of 90 and above;
9. Photocopy of Electric Bill for the last 3 months prior to the date of the application;
10. CHED application form.

GRANTS-IN-AIDE
1. Certification from the NCIP (National Commission of Indigenous People) or OMA (Office of
Muslim Affairs) - as member of the cultural minority group;
2. Certification from the Principal - that he/she belongs to the upper 15% of the 4th year high
school class;
3. Photocopy of Latest ITR of Parents - with gross annual income of not more than
150,000.00;
4. Photocopy of Birth Certificate --- not more than 25 years old at the time of application;
5. FORM 138 – 3rd grading period;
6. CHED Application Form.

STUDENT LOAN
1. Currently enrolled in any of the CHED -priority courses;
2. Photocopy of Birth Certificate - not more than 30 years of age at the time of application;
3. Photocopy of NCAE/NSAT Result - with 75 percentile rank and above;
4. Certification from the school - having a weighted average of 2.5 or higher in the last
semester attended;
5. Certificate of ITR of Parents - with gross annual income of not more than P180,000.00;
6. Not presently enjoying any scholarship or study grant;
7. CHED Application Form.
8. 2x2 I.D. picture – 2pcs.
