COMMUNITY HEALTH PROGRAMS COORDINATOR

Reporting to and working under the direct supervision of the Health Director:
Assist UOI First Nations in the articulation and development of their
needs and priorities as the relate to communit health programs
funded ! the "rovince of Ontario#
Develop and$or enhance UOI funded pro%ects& skills and capacit to
achieve desired communit outcomes#
Oversee the activities related to communit health programs while
ensuring the responsi!ilities and o!ligations of the pro%ect$s are !eing
met#
RESPONSIBILITIES 'includes !ut not limited to(:
Assist) advise) and support UOI communities as re*uired to ensure
participation in health programming#
+upport the implementation and management of funded programs to
increase and maintain capacit and competence in program
governance) program planning) program development) program
management and administration) program evaluation and program
design#
+upport the development and maintenance of an ade*uate
administrative) travel and accounta!ilit infrastructure#
"rovide assistance in preparing accurate$complete documentation for
annual su!missions and ear,end reports#
-oordinate implementation of funding agreement re*uirements)
relating to the A!original Health .a!ies Health -hildren) Responsi!le
/am!ling and Dia!etes 0ducation program including su!missions) and
reporting re*uirements$deadlines#
1eet the re*uirements of funding agencies and UOI 2nancial
guidelines and policies#
0nsure that funding dollars are 3owed to appropriate funded pro%ects#
"rovide communit visits to assist funded pro%ects with reporting and
meeting reporting re*uirements#
Attend meetings as assigned#

*UALIFICATIONS:
+olid working knowledge of the Anishina!ek Nation#
Relevant post secondar education diploma or degree#
Relevant administrative coordination e4perience$skills#
04tensive knowledge$e4perience with 1icrosoft O5ce products
including 04cel and Access
04perience utili6ing and maintaining data!ase sstem would !e an
asset
04cellent oral and written communication skills#
1ust possess valid Ontario driver&s license and !e insura!le#
Availa!le to travel and su!%ect to irregular hours#
SALARY: -ommensurate with e4perience and education#
