waive all my rights to claim or take any legal action against the Bank for reimbursement of such amount.

I further agree that the Bank shall debit my account only when there are moneys remaining in the account. I also waive notice of such
debit because I will be informed of all details from Pass Book/Statement of the Bank or Invoice/Receipt of the Company.

In case my account number mentioned above has been changed for whatever reason, this Request and Consent shall remain in effect
in all respects with regard to the new account number.

This Request and Consent shall be effective immediately and shall remain in full force and effect until each of the Bank and the
Company has received a 30 days prior written notice from me to revoke this Request and Consent.
