JOINT AFFIDAVIT OF LEGITIMATION
We, ROLANDO R. LELIS, and JENNALYN A. LELIS, Filipinos, of legal age, and
residents of Baas, Hilongos, Leyte after being duly sworn to in accordance with law hereby depose and
say:
1. That we are the biological parents of RENZ JEROME AVILA who was born at Town and

Country Village, Brgy. Mayamot, Antipolo City, on August 24, 2003;

2. That the facts of birth of said child has been duly registered on September 1, 2003
under Registry No. 2003-8259, at the Local Civil Registrar of Antipolo City;
3. That at the time of the said childs conception and birth, we were not yet married;
4. That at the time when RENZ JEROME AVILA was conceived, we were not hindered
by any legal impediment to marry each other, and we could have contracted marriage;
5. That we actually contracted marriage subsequently on February 24, 2007 at Hilongos Roman
Catholic Church, Hilongos, Leyte, solemnized by Rev. Fr. Van O. Cabug-os, Parish Priest;
6. That said marriage was duly registered at the Local Civil Registrar of Hilongos, Leyte under
Registry No. 2007-80;
7. That by reason of such marriage, RENZ JEROME AVILA had been legitimated and
bestowed with all rights and privileges pertaining to a legitimate child, including the right to
use the surname of the father such that he is already known as RENZ JEROME A. LELIS;
8. That we execute this affidavit to attest to the truth of the foregoing statements and to cause the
change of the entry of birth as to his status from illegitimate to legitimate.

JOINT SUPPLEMENTAL AFFIDAVIT
(To Supplement Affidavit of Legitimation)
We, RICO D. MASAN and JONISA TUYOR-MASAN, both Filipinos, of legal age and
residents of Marangog, Hilongos, Leyte after being duly sworn to in accordance with law
hereby depose and say:
1. That we are the biological parents of RHEA JEAN TUYOR who was born at
Hilongos District Hospital, Hilongos, Leyte, on June 26, 2010, and whose facts of
birth was duly registered at the Local Civil Registrar of Hilongos, Leyte, under
Registry No. 2010-1549;
2. That during the conception of the said child, I, JONISA TUYOR-MASAN, her
mother was still a minor, I, being still 17 years old then;
3. That we have subsequently contracted marriage on March 8, 2011 in Anahawan,
Bato, Leyte, solemnized by Rev. Fr. Allen T. Arayan, Parish Priest, and which
marriage was registered at the Local Civil Registrar of Bato, Leyte, under Registry
No. 2011-147;
4. That we execute this affidavit to attest to the truth of the foregoing statements to
supplement our affidavit of legitimation previously executed , for the purpose of
supporting the application for the legitimation of our daughter RHEA JEAN TUYOR.
