Poliovirus Immunology: Vaccines,
Problems for the Prevention/Eradication
and Future Interventions
Polio is a contagious disease that is caused by the poliovirus, an enterovirus in the family Picornaviridae. The virus enters through the oral mucosa and
multiplies in epithelial cells of both the oropharynx as the gastrointestinal
tract, releasing virus in oropharyngeal secretions and through the stool. The
mode of transmission is fecal-oral and / or oral-oral. The virus preferentially
infects children under 5 years. Most infections are asymptomatic and self-limiting gastrointestinal tract. Eventually it spreads to the central nervous system
and affects the anterior horn motor neurons of the spinal cord causing paralysis and even death. We will describe host-virus interaction and the natural history of infection which depends on many factors, including the type of viral
inoculum (serotypes VP1, 2 and 3) and host factors, such as nutritional status,
concurrent infections and the ability to induce protective immune responses,
such as, humoral anti-viral antibody responses with neutralizing antibodies,
mucosal immunity and systemic adaptative immune responses. We will discuss the relevant aspects of the immuno-pathogenesis of the infection by
poliovirus and the problems related to the host-virus interactions in the subjects vaccinated, with the latest advances in the strategies to develop optimal
protection with the different poliovirus vaccines that could allow the development of a more effective immunization with induction of the effector mechanisms that would prevent development of the disease, transmission of the
virus, out-breaks and eventually the poliovirus eradication.

la superficie del virin y constituyen los antgenos (Ag) importantes para la induccin
de anticuerpos neutralizantes.
Existen tres sitios principales de neutralizacin antignica (N-Ag) en las VP: N-Ag
I, II y III. El sitio antignico ms frecuente
es N-Ag I (el 90% de los anticuerpos neutralizantes tipo 2 y tipo 3 estn dirigidos
frente a N-Ag I (figura 1).
El poliovirus existe como 3 tipos antignicos o serotipos PV-1, PV-2, y PV-3 (tabla
2). Para todos los serotipos virales la mayora de los eptopos de los sitios N-Ag se encuentran en clusters en VP1. Ha habido poca evolucin antignica en los sitios N-Ag
y los aislados virales no relacionados del tipo-salvaje del poliovirus [wild-type poliovirus (WTP)] varan en uno o ms sitios NAg5 (figura 1). La proteolisis con proteasas

El poliovirus entra en las clulas infectadas a travs del receptor CD155 y comienza
su ciclo biolgico replicativo (figura 2). En
el interior de la clula infectada el RNA del
virus se traduce para dar lugar a una poliprotena cuyo procesamiento conduce a las
diferentes protenas codificadas por el virus. La replicacin de su material gentico
permite la sntesis de ms material proteco
o su encapsidacin para liberar nueva progenie viral6.

Se han propuesto tres hiptesis no excluyentes para explicar su entrada al sistema nervioso central (SNC). Todas las teoras requieren viremia primaria. La
primera hiptesis predice que los viriones
pasan directamente de la sangre al SNC a
travs de la barrera hematoenceflica9.
Una segunda hiptesis sugiere que los viriones son transportados desde los tejidos
perifricos a los que llega a travs de la
sangre, por ejemplo el tejido muscular, a la
mdula espinal, a travs de las vas nerviosas por transporte axonal retrgrado10. Una
tercera hiptesis es que el virus es transportado hasta el SNC a travs de los monocitos o macrfagos infectados6.

Tras un perodo largo silente o asintomtico, la infeccin por poliovirus puede asociarse a una enfermedad secundaria que recuerda la enfermedad neuronal
motora.
PREVENCIN DE LA POLIOMIELITIS:
VACUNAS
La vacuna de poliovirus inactivados trivalente (IPV) diseada por Jonas Salk fue
la primera que consigui una licencia en
1955. El constructo de la vacuna deriva de
cepas seleccionadas de WTP inactivadas
con formalina, cuya potencia inmunognica fue aumentada en 197012,15. La figura 4
muestra a Jonas Salk en una Conferencia
sobre VIH celebrada en San Francisco (6th

International AIDS Conference, San Francisco, United States, 1990) donde present
una vacuna frente al SIDA con un constructo de similar diseo, con un virin
completo inactivado, derivado de cepas de
un paciente con virus de la inmunodeficiencia humana (VIH-1). En la figura aparece tambin el ponente Dr. Eduardo Fernndez-Cruz que colabor activamente
con el Dr. Salk y realiz una investigacin
en Espaa durante el perodo 1998-2007
para el desarroll clnico de la vacuna Salk
con el inmungeno VIH-1, obteniendo resultados sobre su potencial eficacia para
inducir respuestas potentes de la inmunidad celular especfica anti-VIH en pacientes con infeccin crnica por el VIH-116,17.

La evidencia emprica apoya la efectividad de la vacuna OPV. Ha quedado demostrada su eficacia en la interrupcin de
la circulacin del poliovirus. As mismo,
la OPV es eficaz en la prevencin de la
enfermedad paraltica en un 90%15.
La forma trivalente de la OPV es altamente eficaz estimulando la respuesta inmune sistmica con anticuerpos circulantes especficos a todos los serotipos en los
pases industrializados. Sin embargo, en
los pases en vas de desarrollo la eficacia
es slo del 73% y del 70% de los individuos vacunados en cuanto a la generacin
de anticuerpos frente a los serotipos 1 y 3,
respectivamente20, 21.
1) VDPVs inmunodeficientes
(iVDPVs), aislados en sujetos con inmunodeficiencias primarias (IDP) y 2) VDPVs
circulantes (cVDPVs: circulating vaccinederived poliovirus) aislados tras prolongada replicacin del virus de la vacuna. Tales
virus circulantes cVDPV tienen el potencial de causar rebrotes de polio. Durante el
perodo 2000-2009 se reportaron globalmente 12 brotes epidmicos de cVDPV27.
Prevenir la emergencia de los cVDPVs
requiere mantener altos niveles de vacunacin en todos los pases antes de que la
polio sea erradicada globalmente. Por otra
parte, los inmunodeficientes con IDP (con
inmunodeficiencia variable comn, CVID)

En trminos generales, OPV es una vacuna segura aunque la cada de las defensas inmunolgicas en las personas vacunadas (Immunity Gap) puede ser un factor de
riesgo de rebrotes de infeccin importante.
El riesgo de VAPP, cVDPV y de iVDPV ha
llevado a muchos pases a cambiar la relativamente barata y sencilla OPV por la de
mayor coste administrada por va parenteral, la vacuna SALK inactivada contra la
polio (IPV).
Los anticuerpos de mucosa (sIgA) inducidos por las vacunas pueden neutralizar el
poliovirus en el intestino resultando una

reduccin del tiempo de excrecin del poliovirus tipo 1 y 3 en la faringe, en el intestino y las heces.
