In CY 2007, Primary and Secondary Syphilis (P&S) reported in Pennsylvania (exclusive of Philadelphia) saw a slight decrease
over CY 2006: from 139 cases to 134. Eighty-One (60%) of the 134 P&S syphilis cases were reported in three Pennsylvania
counties: Allegheny (51), Bucks (17) and Montgomery (13). These counties, which are adjacent to Philadelphia (i.e., Bucks,
Montgomery) and include the city of Pittsburgh (Allegheny), have significant Men having Sex with Men (MSM) populations.
The breakdown of P&S syphilis by age for CY 2007 (exclusive of Philadelphia) indicates a range of 15-44 years of age, and
with 69 (51%) of the 134 P&S cases reported in individuals 25-44 years of age.
From 1998 to 2007, Pennsylvania experienced a 1311% increase in P&S syphilis: from 9 cases in 1998 to 134 cases in 2007.
From CY 2005 to CY 2007 the average number of reported P&S syphilis cases in the Commonwealth (exclusive of
Philadelphia) averaged more than 126 cases per year (113 cases in CY2005, 139 cases in CY2006, and 134 cases in
CY2007). Concurrently, there has been an increase in P&S case rates in the Men having Sex with Men (MSM) population
throughout the state, and especially in Allegheny County. In 2001, a dramatic increase in cases occurred that was the result of
an outbreak of early syphilis identified in New Kensington, a small town in southwestern Pennsylvania near Pittsburgh, involving
crack cocaine users and commercial sex workers.
Cases with MSM risk factors were identified in twenty (25) counties during 2007. Analysis of the cases identified with MSM risk
factors indicated the following: sex with a male, or sex with multiple male partners, or the use of the Internet to arrange for sex
with a male partner(s). Of the male P&S syphilis cases interviewed in CY 2007, 62.72% (69/110) had sex with a male, 23%
(25/110) arranged for sex with male partners they located on the Internet, and 23% traveled 20 or more miles from home for
male sexual encounters, a practice (especially when crossing state lines) which makes locating contacts more challenging.
Chlamydia
In Pennsylvania, chlamydia became a reportable disease in October 1991. In CY 2006, a total of 39,487 cases of chlamydia
were reported in Pennsylvania (inclusive of Philadelphia). This is an increase of 3.45% (2,938). In 2007, 42,469 cases were
reported.
The breakdown of chlamydia cases by race in 2007 was as follows: 48% (8,649/41,096) white; 24% (16,818/41,096) black;
16% (2,583/41,096) Hispanic; and 12% (13,046/41,096) other/unknown, which includes Asian/Pacific Islanders and American
Indian/Alaska Natives.
The most prominent principal risk factor for chlamydia was age. In CY 2007, 92% (39,247/41,096) of the case reports were for
persons under the age of 35.

Gonorrhea
In calendar year (CY) 2007, there were 7,460 gonorrhea cases reported in the project area (Pennsylvania, exclusive of
Philadelphia). The case rate per 100,000 for 2007 was 69.3/100,000. The number of cases reported in 2007 was the
highest number of reported cases reported for the project area over the last 25 years. Of the 7,460 cases reported in CY
2007, younger segments of the population and racial/ethnic minorities bear a disproportionate disease burden for
gonorrhea infection. In 2007, 15 to 24 year olds accounted for 58% (4,317/7,460) of all confirmed cases. Blacks
accounted for 48% (3,596/7,460) of the total cases, whites for 21% (1,550/7,460) of the total cases, and 31%
(1,967/7,460) of all cases reported race as other or unknown. The disproportionate nature of the disease burden is even
more evident when case rates, expressed here as cases per 100,000 population, are compared by age group, race, or
ethnicity. The CY 2007 case rate for 15 to 24 year olds was 94.27 compared to a decrease of 10.57 for all age groups
combined. The case rates for whites, blacks, and Hispanics are 15.75, 627.80, and 146.33 respectively. In most project
areas, males account for the majority of Gonorrhea cases because they are more likely to have symptoms and volunteer
for examination and testing. Females account for 41.2% (3,076/7,460) of project area cases.
As in past years, gonorrhea continues to be geographically concentrated with 59% of the reported cases in the first 6
months of 2007 being reported in only 5 counties in the state (e.g., Allegheny, Erie, Lehigh, Delaware and Lancaster
counties). In Erie, Lehigh, Delaware and Lancaster counties, the increases have been reported in heterosexual
individuals age 15 to 24. In Allegheny County, the reported increase has been particularly noted in the MSM population.
Of these five counties, Delaware County reported the most significant increase reporting an 80% increase in gonorrhea
from the 513 cases reported in CY 2006 to the 925 cases reported in CY 2007. This majority of the increase was
concentrated to just eight census tracts in the southern part of the county and in particular the city of Chester. As a result
of the increases in reported gonorrhea, the city of Chester has reported a gonorrhea case rate of 1,250 cases per
100,000 population, a rate twelve times higher than the state case rate of 102 cases per 100,000 population and
approximately ten times higher than the national case rate of 121 cases per 100,000 population. Of the 925 gonorrhea
cases reported in Delaware county 55 %(507 cases) were identified in individuals age 15-24. Analysis by Race indicated
that 58% (533 cases) were identified as African-American, 10% (91 cases) were identified as White, and 30% (646
cases) as Unknown or Other. Analysis by gender indicated that 39% of the cases (363) were reported in females and
30% in males (282).
