Rebate request must be postmarked within 30 days of purchase.
2. Read and fill out this form completely. Incomplete forms will not be accepted.
3. Mail this form with the original UPC code from the product package (*if downloaded digitally, write
the license number for your copy of AVG Internet Security 1 User / 2 Year on the line provided

The Visa prepaid card is not redeemable for cash or usable at any ATM. Terms and Conditions apply to the card. Subject to applicable law, a
monthly maintenance fee of $3 (USD) applies, but is waived for the first six months after the card is issued. Your card is issued by MetaBank
pursuant to a license from Visa U.S.A. Inc. Cards can be used at any merchants that accept Visa debit cards. AVG reserves the right to substitute a
check of equal value in lieu of a Visa prepaid card at its discretion.
Offer is valid on purchase of specified product(s) only. Submit this rebate form completely filled out with a copy of the sales receipt and original
UPC code from package (or write in license number for digital downloads). Incomplete forms will be denied. Limit ONE rebate per person,
household, family or address. Customer may only redeem ONE rebate per specified product per 12 month period from date of purchase. Rebate
