Talking to this kind of patient is hard, he is easily distracted to the environment and he seems shy when
talking to us. As time passes by, building trust with each other becomes easier.

Documentation is an important part on being assigned to a patient. Even though we are only assigned on
taking the vital signs we ensure that the results we take from the patient are accurate.

Actions that should be recommended are as followed:

1. Patient should have atleast 8 hours of sleep everyday.

2. They should not be bathe early in the morning.

3. The area must be well secured.

Informed clinical instructor on the recommendations that should


be done.

Shared with the clinical instructor the condition of the patient


while interviewing and also other pertinent data that was
gathered.

Endorsed with staff the vital signs of the patient.

Result/Outcomes of Indicators Used:

Our CI and the nurse on duty gave us our patient with their full trust and we gave them some fulfilling results
with our care.

There is really need to practice communication well especially to psychiatric clients.
