We: ERLINDA B. GALAPON, of legal age, Filipino, widow, and children: MA.
ELIZABETH G. NOMBRE, of legal agae, Filipino, married; ARCADIO JR. B.
GALAPON, of legal age, Filipino, married; MA. ELVIRA G. CASAL, of legal age,
Filipino, married; MA. EVELYN G. ARBAS, of legal age, Filipino, married; and MA.
ELAINE B. GALAPON, of legal age, Filipino, single; all the parties are residents of
Tacloban City, and hereinafter referred to as HEIRS OF ARCADIO L. GALAPON, as
supplement/addendum to the Extrajudicial Settlement of Estate with Waiver executed on
5 March 2003 under Doc. No. 246, Page No. 50, Book XXXIV, s. 2003 of Notary Public
Leo S. Giron,

WITNESSETH: That--1. We are the sole and only legitimate heirs of decedent ARCADIO L.
GALAPON, who died intestate on 17 October 2002, leaving behind SIX (6)
CERTIFICATES FOR SHARES OF CAPITAL STOCKS of BANK OF THE PHILIPPINE
ISLANDS for a total of TWO HUNDRED THIRTY ONE (231) SHARES, specifically
described as follows:

b. Certificate No. 239363
c. Certificate No. 224834
d. Certificate No. 189111
e. Certificate No. 163535
f. Certificate No. 134391

2. The decedent has left no known debts or obligations that have remained
unpaid or unsettled; neither has he left any other heirs entitled to succeed to his estate,
except the herein parties who are his surviving spouse and children.
WHEREFORE, and invoking the provision of Section 1, Rule 74 of the Rules of
Court, the aforenamed heirs do hereby adjudicate unto themselves the rights and
interests to these SHARES OF STOCK, subject only to contingent liabilities to creditors,
heirs and other persons who might have been deprived of lawful participation therein in
accordance with Section 4, Rule 74 of the Rules of Court.
FINALLY, we do hereby and further WAIVE, TRANSFER, ASSIGN AND SET
OVER said SHARES OF STOCK in favor of surviving spouse/mother/holder ERLINDA
B. GALAPON, including all the rights, title and interest of the decedent ARCADIO L.
GALAPON as a subscriber and stockholder of BANK OF THE PHILIPPINE ISLANDS, a
corporation organized under the laws of the Philippines, to the extent of the
aforementioned TWO HUNDRED THIRTY ONE (231) SHARES OF STOCK which we
have adjudicated upon ourselves, and we do hereby request and direct said company to
issue six (6) certificates with the corresponding shares in lieu of the above certificates to
and in the name of ASSIGNEE ERLINDA B. GALAPON.
