Client Profile
Name: Milagrosa Miran

Occupation: Curamed employee
Date of Imprisonment: 6/2/12

Milagrosa Miran was imprisoned last June 2,2012 because of being accused as a drug syndicate all
over the city of Baguio and the near provinces. She was arrested at a fast food chain together with
his brother who is also now in the same Jail. According to her statement, she was accused of being a
drug syndicate because her boss turned her in to jail because of a misunderstanding between them.

Family Medical History


Family members of Milagrosa was reported of some disease such as the following:

Hypertension:
Hypertension (HTN) or high blood pressure, sometimes called arterial hypertension, is
a chronic medical condition in which the blood pressurein the arteries is elevated Blood
pressure is summarised by two measurements, systolic and diastolic, which depend on
whether the heart muscle is contracting (systole) or relaxed between beats (diastole) and
equate to a maximum and minimum pressure, respectively. Normal blood pressure at rest is
within the range of 100-140mmHg systolic (top reading) and 60-90mmHg diastolic (bottom
reading). High blood pressure is said to be present if it is persistently at or above
140/90 mmHg.

Asthma:
Asthma (from the Greek , sthma, "panting") is a
common chronic inflammatory disease of the airways characterized by variable and recurring
symptoms, reversible airflow obstruction and bronchospasm. Common symptoms
include wheezing, coughing, chest tightness, and shortness of breath.
Asthma is thought to be caused by a combination of genetic and environmental
factors. Its diagnosis is usually based on the pattern of symptoms, response to therapy over
time and spirometry. It is clinically classified according to the frequency of symptoms, forced
expiratory volume in one second (FEV1), and peak expiratory flow rate. Asthma may also be
classified as atopic (extrinsic) or non-atopic (intrinsic) where atopy refers to a predisposition
toward developing type 1 hypersensitivity reactions.
Treatment of acute symptoms is usually with an inhaled short-acting beta-2 agonist (such
as salbutamol) and oral corticosteroids. In very severe cases, intravenous
corticosteroids, magnesium sulfate, and hospitalization may be required. Symptoms can be
prevented by avoiding triggers, such as allergens and irritants, and by the use of inhaled
corticosteroids. Long-acting beta agonists (LABA) or leukotriene antagonists may be used in
addition to inhaled corticosteroids if asthma symptoms remain uncontrolled. ] The prevalence
of asthma has increased significantly since the 1970s. In 2011, 235300 million people
globally have been diagnosed with asthma, and it caused 250,000 deaths.

Cardiovascular disease (also called heart disease)

Is a class of diseases that involve the heart, the blood vessels (arteries, capillaries, and veins)
or both.
Cardiovascular disease refers to any disease that affects the cardiovascular system, principally
cardiac disease, vascular diseases of the brain andkidney, and peripheral arterial disease. The
causes of cardiovascular disease are diverse but atherosclerosis and/or hypertension are the
most common. Additionally, with aging come a number of physiological and morphological
changes that alter cardiovascular function and lead to subsequently increased risk of
cardiovascular disease, even in healthy asymptomatic individuals.
Cardiovascular disease is the leading cause of deaths worldwide, though since the 1970s,
cardiovascular mortality rates have declined in many high-income countries. At the same time,
cardiovascular deaths and disease have increased at a fast rate in low- and middle-income
countries. Although cardiovascular disease usually affects older adults, the antecedents of
cardiovascular disease, notably atherosclerosis, begin in early life, making primary prevention
efforts necessary from childhood. There is therefore increased emphasis on preventing
atherosclerosis by modifying risk factors, such as healthy eating, exercise, and avoidance
of smoking tobacco.

Client Present Medical State:


The client stated that in the past years of her stay in the jail, she had been experiencing some
illness that she acquired from other prisons. One of which is typhoid fever, she suspected that
it was because of the water that they are using in the area so she tried to seek some help on
some of the personnel of the jail and she was given attention. She was treated by medical
interventions; she stated that the intervention was effective enough to remove the symptoms
that she felt while she was having the illness.
Now on her present condition, she just stated that she is farsighted. So what is
farsightedness?
Farsightedness, or hyperopia, as it is medically termed, is a vision condition in which
distant objects are usually seen clearly, but close ones do not come into proper focus.
Farsightedness occurs if your eyeball is too short or the cornea has too little curvature,
so light entering your eye is not focused correctly.
Common signs of farsightedness include difficulty in concentrating and maintaining a clear
focus on near objects, eye strain, fatigue and/or headaches after close work, aching or burning
eyes, irritability or nervousness after sustained concentration.
Common vision screenings, often done in schools, are generally ineffective in detecting
farsightedness. A comprehensive optometric examination will include testing for
farsightedness.
In mild cases of farsightedness, your eyes may be able to compensate without corrective
lenses. In other cases, your optometrist can prescribe eyeglasses or contact lenses to optically
correct farsightedness by altering the way the light enters your eyes.

Observation:
According to what I have observed, the client is demonstrating some defense mechanism for
her to protect herself from being interviewed or asked of some important, I have seen that she
is used to trying to change the topic when asked of a particular question. Furthermore, she

was trying to make herself clear that there was nothing wrong with the thing that she done
that is why she stated that the police doesnt have the right of arresting her and bringing her
in jail. She also has Reaction-Formation with some other jail members. She told me that she
was doing such because she just wanted to blend in with the other family in the area.
What I did was to explain some matters regarding the possible effects of the things that she is
doing so I tried to persuade her of not doing the things that she is used to like being Plastic
because it might give her problem someday in her life. I also encouraged her to talk to his
family for them to have a regular visit to serve as a support group while she is still in jail. And
most of all, encouraging him to pray to God, which is the best way to release her feeling of
longing and sadness. So far, my patient was in a good condition, she was not demonstrating
something that is weird except that she has learned to use these defense meachanisms for her
to survive the jail life.
