JOHN A. JUAN

I, John A. Juan of legal age, married, and living at Baculud Iguig,, petitioner in this case, state
under oath as follows:
PRELIMINARY STATEMENT
The person examining me is Atty. Kimberley Matammu with address at Carig Sur
Tuguegarao City. The examination is being held at the same address. I am answering her
questions fully conscious that I do so under oath and may face criminal liability for false
testimony and perjury.
This affidavit/testimony of petitioner John Juan is being offered to prove that He is the registered
and lawful owner of a parcel of land located at Casablanca Cagayan covered under Transfer
Certificate of Title No. T-12345. The petitioners testimony is also offered to prove that
defendant Jaime Tuo acting fraudulently and under false pretenses sold the lot, to a certain Four
Dy.

A. I am John Juan, 45 years old, farmer and residing in Baculud Iguig, Cagayan

A. Yes maam.

Q. Do you know a certain Jaime Tuo?

A: Yes maam. He was the one who sold the said lot, registered under my name, to Mr. Four
Dy
4. Q: Do you have any proof of the said registration?
A: Yes Maam. Transfer Certificate of Title No. T-12345 (Exhibit A)
5. Q: How did you know that Jaime was able to sell the said parcel of land to Four Dy?
A: Katibayan ng Orihinal na Titulo Blg. P-78913 was generated.
