Lunch on your own. Meatball grinders, pulled pork sandwiches, chips and drinks will be
available for purchase at a cost of $7.00 per person that day. For planning purposes make
sure to check off on your registration form if purchasing lunch.

*T-Shirts will be handed out to the 4-H YOUTH Participants only. All pre-registered youth will be guaranteed a
T-Shirt. The T-shirts will come in the following sizes: Youth L or Adult: S M L XL XXL.

All youth must be accompanied by parent/guardian or chaperon.
Health Forms for Youth Not Accompanied by Parent
If a parent or guardian will not be accompanying a youth participant to 4-H Dairy and Beef Day, the chaperon
accompanying the youth must have a 4-H Member/Volunteer health form in his or her possession for each
participating member not accompanied by a parent or guardian. The health form must be signed by the youths
parent or guardian.
