Per RRC Inspector San Sein 3/3/2015 06:45 - A house explosion
occurred at 9430 Eloise St, Dallas, Texas. According to Dallas
Fire Rescue a woman and her grandson made it out safely. Both
were later taken to a hospital for precautionary observations. A

total of 5 homes were evacuated by the DFR.
Atmos conducted post incident leak survey and bar hole testing(a). 1500 BLK of Memory Ln to 9500 Blk of Musgrave Dr (b). 9400
BLK of Eloise St to Lisa (c). 9500 BLK of Eloise St to 150 feet
East. The soil conditions were wet due to ice, snow and rain.
Hazardous natural gas (Grade-1) was found at service tap to 1504
Memory Ln. with 100% natural gas reading. The service tap was
located approximately 6 ft from 9430 Eloise property line.
Hazardous condition was eliminated around 2:44 AM on 3/3/2015.
After the excavation of the leak at the service tap, it was observed
that a 1-Inch steel service line has been separated from the 3Inch, coated steel main saddle tap connection fitting (See Photo 1
&2). Also new asphalt street patch was observed on the east of
leaking service tap (See photo 3&4). The marking on the patch
indicated as 8 water main. The normal operating pressure on the
gas main was 20 psig. CP reading on main was -1.04 Volts.

3/3/2015: 3:20AM: Atmos re-checked the sewer vents, no natural
gas concentration were detected.
The odorant test was conducted by Atmos, witnessed by Lt. Chris
Kerr (DFR) and San Sein (RRC). It showed STRONG Odor
Intensity at 1% gas to air mixture.
9430 Eloise St Info: Pier and beam foundation wood frame home
No natural gas service to residence since January 27, 2014 per
Atmos service record.The vacant meter loop to the residence was
retired on 3/3/2015. No natural gas leaks were found on service
tap or service line to the meter. Pressure tested -Inch service
line to 20 psig for 5 minutes from the service tap to the vacant
