Seeds of Hope Co-op Program

Low-Income Subsidized Membership Agreement


Co-op share and value A co-op share is an assortment of freshly-picked locally-grown vegetables and
fruits produced both by Seeds of Hope Farm and its partnering farms that is placed in a box for
distribution to farm members. A wide variety of vegetables, herbs, and fruits will be in production to
serve the nutritional needs of our community. The contents of a share will be the same for all farm
members. A shares value is planned to average, on a weekly basis, a local growers prevailing market
value of $20.00.
Farm member - One who has subscribed to the Co-op and receives a weekly or bi-weekly share. Farm
membership has two tiers: a subsidized tier of qualifying low-income members who pay $12 per share,
and a tier of sponsoring members who pay a higher share price to allow the farm to offer a 40%
discount to low-income members.
Co-op term - The twenty-five (25)-week Co-op term will begin on May 14, 2015 and end on October 29,
2015.
Membership Guidelines: As a member, I understand and agree to the following:

Seeds of Hope Farm, which is operated by Community Action Agency of St. Louis County, Inc.
(CAASTLC), will act in good faith to provide produce for the duration of the prescribed Co-op term, but
there is no guarantee of quantities or contents of the weekly shares.

I am aware that a printed co-op newsletter will be distributed to me each week as part of my share. The
newsletter will be a source of information about events hosted at the farm, our classes and other
gatherings, and broader gardening opportunities. It will feature nutrition information, recipes and meal
plan ideas for seasonal farm ingredients. I have the option of reading this information online at the
Seeds of Hope Farms blog (http://seedsofhopefarm.org/) and may opt out of receiving a paper copy by
contacting the farm manager.

I am responsible for picking up my share. The following rules apply for picking up shares:
a) Shares will be distributed from the Seeds of Hope farm-site at 1120 Trampe Avenue in
Spanish Lake as well as from CAASTLCs main office in Overland and at CAASTLCs BelRidge site. Shares will be made available each Thursday with the exception that the Biweekly shares will be made available every other Thursday. Signs will be set out on the farm
premises to indicate directions on locating and collecting shares. See Preferred place and
time of pick-up options for specifics on each location.
b) If I am unable to pick up my share, I will arrange for someone else to pick up my share. If
my share is not collected on pick-up day during the hours prescribed below for each pick-up
site, I understand that CAASTLC may distribute my share to other customers. Collection of
a share on a day other than the prescribed pick-up day requires advance arrangement with
the farm manager.
c) Alternate places or times for pick-up may become available, pending a written agreement
with the farm manager.

d) I will notify the farm manager at 314.566.8643 or ghahn@caastlc.org in advance if I am not


able to pick-up my share.

e) If in a given week I do not pick-up my share and do not notify the farm manager by phone or
email that I will not be picking-up, no shares will be made for me until I have contacted the
farm manager. Seeds of Hope Farm encourages you to continue your membership and
resume participation.

Farm Management may cancel membership at any time for disagreeable or unruly behavior, including
but not limited to abuse or behavior that farm management views as disruptive to a community
atmosphere.

Low-Income Subsidized Share


The cost of a low-income subsidized share is $12.00. Payment is made only at the time of pick-up of the share.
Subsidized shares are available for purchase by area residents whose household income is at or below 125%
of the federal poverty level. Please read the income table at the bottom of this application to determine if your
household qualifies.
Low-Income Subsidized Bi-weekly Share Option
Bi-weekly members pick up a share once every other (alternating) week. An A week and a B week will be
assigned to each bi-weekly member. Each member will receive a listing of the A and B week schedule. The
schedule will also be posted on our website at http://seedsofhopefarm.org. Week A will begin on May 14,
2015 and week B will begin May 21, 2015.
Work share
A member may reduce the cost of a share and learn more about farming by volunteering on the farm. The
cost of each share may be reduced on a weekly basis by $4.00 as compensation for two (2) hours of field
assistance or $8.00 as compensation for four (4) hours of field assistance. A member can earn a free share
for five (5) hours of field assistance in a given week. Volunteerism must be scheduled in advance with the
Farm Manager.
Methods of Payment
Payment for a subsidized share is made at time of pick-up of the share. Pre-payments for subsidized shares
are not accepted.
Accepted methods of payment are cash, money order, check, credit/debit, and EBT. Make check or money
order payable to CAASTLC.

Payment by cash, check, money order or EBT will be accepted until 8:00 PM. Payment by credit/debit will not
be accepted at this location.

Waiver and Release & Authorization to Photograph and/or Videotape


I agree to waive and release CAASTLC and their officials, employees, agents and insurers from any claim, loss
or liability for death, personal injury, or loss or damage to personal property arising out of my use of the Seeds
of Hope Farm unless arising out of CAASTLCs and//or Owners intentional wrongful acts. This waiver and
release shall be governed by Missouri law.
I authorize photograph(s) and/or videotaping to be taken of me by CAASTLC solely for the purposes of
promotion, documentation, and/or possible publication in newspaper(s), CAASTLC report(s), CAASTLC inhouse materials, and/or the general media. I understand that neither I nor anyone else in my family will receive
compensation from CAASTLC for such photograph(s) and/or videotaping. The use of all such photograph(s)
and/or videotaping shall be the sole and exclusive property of CAASTLC.
