House Calls: Caring for Dementia Patients and their Caregivers at Home
The Mount Sinai Visiting Doctors Program (MSVD) is the largest academic home based primary
care practice in the country, caring for over 1500 patients annually. This presentation will
address the primary care model and how it is instrumental in detecting and treating all aspects
of dementia, and will showcase the innovative ways that the team addresses the most
commonly identified issues of its patients with dementia.

Panel: Supporting Caregivers and Their Care Recipients, featuring:


A Vision to Assure Quality Support for Geriatric Veterans
The Caregiver Respite Group is an innovative model of care blending medical
healthcare and social daycare for older veterans with early dementia and their families.
Building on the vital sense of camaraderie, the team works to meet the physical, mental,
and social needs of participants.

Private Troubles; Public Issues: Transforming Personal Experience into


Macro-level Advocacy on Behalf of Caregivers
Dementia and caregiving for people with dementia is noted as a public health issue by
this panelist, who will speak about the connection between personal experience as a
caregiver and work involving policy makers and local government in providing support to
other caregivers.

An Academic-Community Partnership Supporting Dementia Caregivers


This presentation describes a partnership among Columbia University Medical Center, New
York University, Burgio Geriatric Consulting, and Riverstone Senior Life Services that integrates
dementia caregiver counseling interventions with social work services in the community of
Northern Manhattan.

A Community-Based Participatory Research Video to Address Memory and Aging


Knowledge Gaps Among Latino Older Adults
The presentation describes a Community-based Participatory Research that identified the
cognitive health literacy needs and preferred health communication methods of Spanishspeaking/low English proficiency Latino senior center participants in East Harlem. Findings
informed the collaborative production of a health-education/health literacy video entitled Que le
Pasa a mi Memoria? (What is Going on with my Memory?). During this workshop participants
will watch the collaboratively produced video and will obtain information on how to use
collaborative video production methods as a tool to promote engagement and participation in
social work services.
