betweenhighandlowenergyinjuriesandthe
conceptofadequacyofsofttissuecoverinthehighenergy
injuries.
Thesiteofthefracturehasanimportanteffecton
theinjurygrade,particularlywithregardtoquantity
oflocalmusclecoverandthevulnerabilityofthelocal
softtissuetoinjury.Afractureinthenormalfemur
hastobehighenergybecauseoftheintrinsicstrength
ofthebone;accordinglyallopenfemurfractures
mustbegradeIII,althoughthethickmusclecoverof
thethighusuallyprovidesadequatesofttissuecover.
Incomparison,thetibiaisuniquewithregardtoits
poorsofttissuecoverandparticularvulnerabilityto
injury.Accordingly,itisdifficulttohaveagradeIIIa
injurytothetibialdiaphysiswheremosthighenergy
diaphysealanddistaltibialfracturesaregradeIIIb
andrequirecomplexreconstructivesurgery.Themost
severeinjury(IIIc)istheopenfractureassociatedwith
avascularinjurythatrequiresrepair.Thesecanoccur
witheitherasevereorveryarelatively'minor'would.
Thehighgradingisbecauseoftheprimaryrisktothe
limbassociatedwiththearterialdisruptionandthe
susceptibilityofischaemicmuscletoinfection.
Asimilarclassificationsystemforclosedfractures
hasbeenproposedbyTscherne.4Thisalsodescribes
therangeofsofttissueinjuriesfromessentiallyno
significantinjurytomajorvasculardisruption,severe
