GENERAL INSTRUCTIONS

ELIGIBILITY FOR OPTIONS

All in service civil officers/officials (BPS 1 to 16) of the Federal Government working in Ministries / Divisions / Attached Departments
and Sub-ordinate Offices including civil officers/offcials paid from defence estimates and employees of Federal Government
Autonomous Bodies / Corporations and other Federal Govt organizations in BPS 1 to 16 or equivalent on or after July 1, 2007 are
eligible to be benefited from this housing programme going to be launched by Pakistan Housing Authority. However, quota for such
autonomous bodies etc. will be fixed.
Where both husband and wife are eligible officers/officials, only one will be benefited from this scheme.
Officers/officials will be entitled to only one housing unit in his / her service life time under this housing scheme located anywhere in
the country.
Spouse of the in-service deceased oficer or retired deceased officer/offical after July 01, 2007 are also eligible.
The officers/officials working on contract/adhoc basis/Contigent paid/work charged shall not be eligible.
Armed Forces JCO's & ORs of defence services shall not be eligibile.
The officers/officials who have been dismissed from Govt. service on disciplinary grounds or given a major penalty and resignation
or retirement of officers/officials without terminal benefits shall not be eligible.
The oficers/officials who own a plot / house / apartment in their name or in the name of their spouse/dependent children having been
allotted by the Federal Govt. Employees Housing Foundation or any other Federal / Provincial Government agency whether retained
or disposed off shall not be eligible.
The options for choice of cities once exercised will not be changed and will be treated as FINAL.

OPTION FORM TO BE USED


Following OPTION FORM shall be used:
Federal Government officers / officials (BPS 1~16) shall apply on option
form

Option forms can be obtained from Pakistan Housing Authority, Head Office, Ground Floor, Shaheed-e-Millat Secretariat Islamabad
free of cost.
Option form can also be downloaded from the web-site of Pakistan Housing Authority at www.pha.gov.pk
Photocopies of the Forms can be used. Use A4 size paper for photocopying. While making a photocopy, DONOTREDUCEOR
ENLARGEthesizeoftheform.

SUBMISSION OF OPTION FORM


Officers/officials shall submit their filled-in forms PHAF-03 duly verified by the Drawing & Disbursing Oficer (DDO) of their present
posting ofices, to Pakistan Housing Authority, Ground Floor, Shaheed-e-Millat Secretariat, Jinnah Avenue, Islamabad.

INSTRUCTIONS FOR FILLING-IN OPTION FORM

Please type or print in CAPITAL letters.

b.

All entries in the forms must be duly filled in English.

c.

No cutting / overwriting or ambiguous entries are acceptable.

d.

Latest Photograph shall be pasted on Option form where specified.

e.

Photo copies of CNIC (both front & Back) shall be pasted in relevant place.

f.

Fee of Rs.200 in shape of demand draft / Pay order (drawn on any branch of same bank in Islamabad) should be enclosed.

Photograph, photocopies of CNIC shall be pasted with gum. Stapled/pin up option form will not be accepted.

i.

Option form should be counter-signed by the authorized officer of the present office.

j.

Please bring photocopy of duly filled option fom for submission by hand for receiving purposes.

Signatures on application should match with Computerized National Identity Card (CNIC)

The last date for receipt of Option Forms is April 30, 2008
The applicants who do not submit their options by the target date, can submit their option forms thereafter but their names will not
be included in the present options list.
