FOR SUPPLIERS IN METRO MANILA

Completion of the registration process is a pre-requisite to being an accredited supplier of San Miguel
Corporation and its businesses. The steps in registration, required documents and the supplier registration
form are included in this set.

Registration Guidelines

FORMS AND DOCUMENTS


Upon receiving the Supplier Registration Set from the buyer, completely and correctly fill out the Supplier
Registration Form and prepare the required supporting documents. Make sure that the Supplier
Registration Form is endorsed and signed by the designated buyer.
Below is the list of required documents for registration. You must bring the original copies of the
documents upon submission for validation.
FOR SUPPLIERS IN METRO MANILA
I.
Duly accomplished Supplier Registration Form (endorsed and signed by the buyer)
II.
Original and photocopy of the following:
1 Business Registration
For corporations:
o SEC Registration and
o General Information Sheet
For sole proprietorships:
o DTI Registration and
o Mayors or Business permit
o Any of the following proof of ID:
Drivers license/ NBI/ Passport/ or any other government issued ID
2 Any proof of business address, such as:
Proof of occupancy
Lease agreement
Utility bills
3 Audited Financial Statements for the past three (3) years or Financial Forecasts for three (3) years for new
companies
4 BIR Form 2303 or BIR Certificate of Registration
5 For Corporations, a Table of Organization (with names and positions)
6 Official Receipt of registration payment
7 Additional documents for contractors: (Construction, Civil, Mechanical, Electrical Works):
Philippine Contractors Accreditation Board (PCAB License)
8 For Manpower Service providers:
DOLE Registration

PAYMENT
Pay the Php 2,000.00 registration fee. Mode of payment may be cash/check at the:
SMC Treasury Cashier

*Note: For Check Payments, Payees Name should reflect SAN MIGUEL CORPORATION

SUBMISSION
Present official receipt of registration fee payment, and submit the duly accomplished Supplier
Registration Form together with the required attachments to:

REMINDERS
1 The registration and evaluation process approximately takes fifteen (15) working days.
2 You are required to provide an active e-mail address so that you can be notified of the status of
your application.
3 Only applications with complete requirements will be processed.
