INTRODUCTION:
Hospitals deal with the life and health of their patients. Good medical care relies
on well-trained doctors and nurses and on high quality facilities and equipment.
Good medical care also relies on good record keeping. Without accurate,
comprehensive and up to date and accessible patient notes, medical personnel
may not offer the best treatment or may in fact misdiagnose the condition,
which can have serious consequences. Associated records, such as x-rays,
specimens, drug records and patient registers, must also be well cared for if the
patient is to be protected. Good records care also ensures the hospitals
administration runs smoothly; unneeded records are transferred or destroyed
regularly, keeping storage areas clear and accessible; and key records can be
found quickly, saving time and resources. Records also provide evidence of the
hospitals accountability for its actions and they form a key source of data for
medical research, statistical reports and health information systems .
Managing Hospital Records addresses the specific issues involved in managing
clinical and non-clinical hospital records. A comprehensive records management
system in a hospital helps to ensure that staff have access both to clinical
information and to administrative records on a wide range of issues, including
policy, precedents, legal rights and obligations, personnel, finance, buildings,
equipment and resources.
PROBLEM STATEMENT:
Records Management refers to an on-going process of managing the records in a
media neutral basis in accordance with approved policies, procedures and
schedules. Records Management as a discipline defines and applies business
rules related to the creation, protection, retrieval and disposition of an
organization as records over time. Retention schedules are the cornerstone of a
successful Records Management process.
Keeping track of all the activities and their records on paper is very
cumbersome and error prone. It also is very inefficient and a time-consuming
process Observing the continuous increase in population and number of
people visiting the hospital. Recording and maintaining all these records is
highly unreliable, inefficient and error-prone. It is also not economically &
technically feasible to maintain these records on paper.
PROBLEM SOLUTION:
Keeping the working of the manual system as the basis of our project. We
have developed an automated version of the manual system, named as
Hospital Management System.
The main aim of our project is to provide a paper-less hospital up to 90%. It
also aims at providing low-cost reliable automation of the existing systems.
The system also provides excellent security of data at every level of usersystem interaction and also provides robust & reliable storage and backup
facilities.

The application will be windows-based, self-contained and independent
software product.

The application will have a user friendly and menu based interface. Following
screens will be provided.
A. A Login Screen for entering username, password and role
(Administrator, operator) will be provided. Access to different screens
will be based upon the role of the user.
B. A Form for Search the details of a patient.
C. The Form for creating a new patient record will contain text fields
where the Patient ID will be machine generated and the rest of the
details will have to be filled up.
D. A Form for generating the tests reports.
E. The Form to produce a bill will create fields such as Patient ID,
Appointment No., Doctors charges, Hospital charges etc. which will
need to be filled up.

SYSTEM FEATURES
Login module
Description
This module records only user and password of the user.
Patient module
Description:
It keeps track of all details about both in-patient and out-patient. Patient id,
patient name, address, admitted date, doctor name, and room no are
entered in a form and stored for future reference. Also particular patient
details can be viewed in the table using a separate form with an attribute
patient id.
Inpatient module
Description:
Admission request will be made here. Request for admission is made before
patient admitting the hospital.
Outpatient module
Description:
This module manages activities related to patient who visits the Hospital or
Resident Doctor or Consultant Doctor for Medical Consultations, diagnosis
and treatment.
Billing module
Description:
This module bills the both inpatient and outpatient who comes to hospital. It
also includes Payment details of patients. Depending on the payments bill
report is generated.
Task module
This module is used by system admin to manage the system. It has following sub
modules.
Add New Patient :- This module is used to add a new patient.
View Details :- This module is used to view the details of a previously admitted
patient.
Search :- This module searches the record of desired patient using his ID, Name,
City or Blood Group.
Delete :- This module deletes the entry of a patient.
Recycle bin :- This module retrieves the deleted patient data.
