A Guide to fill up TEJAS - BSNL RMA Form - 2013

Please fill up the green boxes in the Blank RMA Form , by giving the following details :
RMA Requester Name : [ Mandatory ] Please fill Name of the individual who will take responsibility for the cards
Date of Request : The date when the filled up RMA form will be sent by email to Tejas
Company : The BSNL Division or Circle name
Address : The official address of the RMA Requester Name // It may also be same as shipping address
Telephone No. : Land Line or any Official Number
Mobile No. : The mobile number of the person who will coordinate with Tejas for the repair of these cards.
Email ID : Valid Email ID of person who will coordinate the repair of these cards.
Address for Return Shipment (After Repair) : It may be same as above address.
TIN No. (should be exactly 11 digits) : It can be obtained from your accounts officer
Card Details : Tejas Part No. (PN) and Card Serial No. (SN) : This is found on a white sticker on the PCB ( example PN:
111-PCA000021-E // SN: AC-3409-16-1962 ). If unable to find PN number, mention Product/Card Details in RMA form.

Card Description (Name Printed on Front Plate of Card) :

Fault Description : Describe in about Twenty words the suspected fault in the functionality of the card.

A Brief View of the BSNL RMA Process :


Step 1 ) Perform testing of your card & confirm correctly the suspected fault in the card.
Step 2 ) You should make a Request for a fresh Blank RMA form, from Tejas

Send a blank email with subject line content as 41719090 to bsnlrma@india.tejasnetworks.com

You will receive immediately a response email with two attachments - RMA Form & a Guide to fill RMA Form.

Step 3 ) Please download the attachments & read through the detailed instructions.
Step 4 ) Fill up the RMA form as per instructions in the guide.
Step 5 ) To the same email, send a reply email with attachment of the freshly filled RMA form (do not alter subject line).
Step 6 ) You will receive one more response email immediately, mentioning the receipt of your filled up form.
Step 7 ) We will take approximately 2 days to process the entries in the form & check the technical correctness.
Step 8 ) We will next reply with Estimate mentioning the approx repair charges.
Step 9 ) You have to reply to this email with an Approval/Acceptance of Estimate. The acceptance email should be
from DE/SDE level. It can be in email writing or Scanned copy approval. (do not alter our subject line)
Step 10 ) After we receive your Acceptance Email we will allot the RMA Number, will email back to you the same form.
Step 11 ) Kindly proceed forward for couriering of card to the address mentioned in the RMA form.
Step 12 ) Typically we need 21 working days to repair the card from date of receipt of the card.
Step 13 ) After the repair is complete, we will prepare Pro-forma Invoice indicating the charges.
Step 14 ) Hard Copy of Pro-forma will be couriered to your address.
Step 15 ) You have to process the Pro-forma Invoice with your accounts department & follow up with them for payment
dispatch.
Step 16 ) Once the payment has been realized in Tejas account, we will prepare a Final/Service Invoice and we will
dispatch the Repaired card to your postal address mentioned in RMA form.
Step 17 ) Once the card is received by you, you may submit the Final Bill and close the case with your accounts dept.
