I, NICHOLAS A. BERMUDO, of legal age, Filipino, married to Elenita B.

Bemudo, and with postal address at 78 Lopez Jaena St., Tacloban City, after
having been sworn according to law, depose and say: THAT--1. I am the sole and only surviving heir of decedents PERFECTO
AGUILAR BERMUDO and ROSARIO ALCANTARA BERMUDO, who died
intestate on May 26, 2010 and March 4, 1991 in Tacloban City respectively,
leaving certain real property in Tacloban City and more particularly described as
follows:

S.53-48 E., 6.15 m. to point 4; S.37-16W., 9.66 m. to point 5;
N.52-33 W., 5.87 m. to point 6; N.41-37 W., 10.45 m. to point 7;
N.71-25 W., 2.63 m. to point 8; N.30-43 E., 7.77 m. to point 1;
point of beginning.
Containing an area of ONE HUNDRED AND SIXTY NINE
(169) SQUARE METERS, registered in the name of PERFECTO A.
BERMUDO, married to ROSARIO ALCANTARA.
All points are marked on the ground by B.L. cyl. Conc. Mons.
Bounded on the N., along line 1-2 by Calle Pizarro (now) Lopez
Jaena St.; on the NE., SE., and SW., along lines 2-3-4-5-6-7-8 by
Lot 3, Cad-65; and on the W., along line 8-1 by Lot 30, Cad-65.
Bearings true.
This lot was surveyed in accordance with law and existing
regulations promulgated thereunder, by Henry f. Nerbert, Cadastral
Land Surveyor, from Oct. 26-30, 1911 and approved on Jan. 16,
1914. Note: This land is covered by M.S.A. No. (VI-4)41.
2. The decedents have left no known debts or obligations that have
remained unpaid or unsettled; neither have they left any other heirs entitled to
succeed in their intestate estate, except the herein affiant who is their sole
surviving son/child.
WHEREFORE, and invoking the provision of Section 1, Rule 74 of the
Rules of Court, the herein affiant does hereby adjudicate unto himself the entire
estate hereinabove described, subject only to contingent liabilities to creditors,
heirs and other persons who might have been deprived of lawful participation
therein in accordance with Section 4, Rule 74 of the Rules of Court.
