STAFF SELF-ASSESSMENT QUESTIONNAIRE


A) ACHIEVEMENTS - How successfully do you feel you met your objective for the last 12
months.
Please note and comment on the achievement of your agreed objectives over the appraisal
period, and what additional achievements you may have made. Use this section to raise issues
about difficulties, changes that have had an impact on your level of achievement.
1. What were your key objectives over the last 12 months and how well do you feel you did in
achieving them
To learn as many inspection method I can and so far I would not say that Ive 100% achieved it but
maybe 70%

2. What problems or challenges did you encounter, and how did you overcome them? Are
there still barriers to achieving your objectives
Mostly equipment break down and to overcome it I fully prepare myself with knowledge of the
equipment and bring all the important tools and spare part to overcome it. And some times I do
face problem with dealing with clients and to over come it ill seek advise from my co-ordinator
and the have discussion with the clients.
3. What do you feel you specifically excelled at
Computed Radiography ,C-Scan,EMA and Incotest
4. Do you feel you are fully competent in all aspects of your job?
Yes
5. In which areas do you think you could improve your performance
I need to improve on my reports and currently I am working on it and there is an improvement

The work load has increases,been travelling more often

Ive improve a lot on the technical aspect,and also reporting
B) COMPETENCY - How do you rate your own skills?
Please make honest notes on how you assess your abilities and skills. Your line manager will
then discuss them with you and help you to have a clearer picture.
1. What do you feel are your strengths and weaknesses and how do you think they have
developed since you joined the company
Well I feel my strength is working on side(hands on job) and weakness maybe on the report
side(which I believe Ive over come it)

2. What do you find most interesting in your job? Do you prefer to work on your own
initiative or do you prefer to have systems, rules and guidance?
I got the chance to learn different kind of inspection method,working offshore and I prefer to work
on my own
3. Do you feel you give your clients/inspectors/vendors clear and efficient verbal and written
feedback/instruction wherever possible?
Yes I do.
4. Do you feel that you help to provide a standard Velosi service to clients, or do you strive to
provide the best possible personal service to your clients
Yes ,I always try my level best to provide them excellent service to them
5. Do you feel that you prioritise your work in the best possible way from the point of view
of the company?
Yes
6. Do you think you are sufficiently financially aware of the companies profitability, in terms
of the best use of your time and optimising the profit on the jobs under your control
Yeah when iam on site ill try to promoted /marketing for velsoi to the client to try different other
method and when iam in the office ill try to learn some other inspection methods to improve
myself
7. Do you think your performance could improve if you were managed better
Currently iam happy with the way iam managed

C) PERSONNAL ISSUES

matter
Even though its a bit far for to come to but Ive no problem with it. I am happy

Yes it is
3. How well do you get on with others in your team and the company
Quite good
4. How flexible, punctual, reliable and committed do you feel you are
Good
5. Do you feel the company conducts a fair approach to personal issues such as Annual
Leave, Tax, Sick Days, Pensions etc(if applicable)
Ive no comment but during public holidays and if we are working on site we dont get redeem it
when we are back..

D) ASPIRATIONS In what areas could you develop your performance over the next 12
months and how could this be achieved
Please note what you expect your job will involve over the next 12 months. This will form the
basis of a discussion, which will lead to agreed objectives for the coming period.
1. Provide some suggested objectives
Company should provide training(which they had did last year),and maybe on TQM(total quality
management )
2. Are there specific aspects of your work in which you would welcome further guidance and
training
Yeah, I need proper certification to meet the NDT requirement .
3. Do you feel that clear Job Descriptions and Reporting Lines for all staff would aid the
development of your performance
Yes(iam happy with the current system)
4. What are your expectations in terms of how the company should be implementing your
training (e.g. what do you feel co-ordination training should involve should it be more
structured)
Company should send us for proper training before sending us on site for job.
5. Do you feel there are any external training courses that would help you considerably with
your job, and also be financially worthwhile for the company
Yeah because those instructor who are going to train us,they professional and experience and we
can learn a lot from them to improve our self.
6. Do you think the relationship between your office and different Velosi Offices (outside
Europe) is an issue and if so how could it be improved
