Personal appearance
Confirmed appointment
Birth Certificate (BC) in Security Paper (SECPA) issued by the National Statistics
Office (NSO) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar (LCR)
and duly authenticated by NSO. Transcribed Birth Certificate from the LCR is required
when entries in NSO Birth Certificate are blurred or unreadable. (Report of Birth duly
authenticated by NSO if born abroad)
No need to submit a passport size photo
Valid picture IDs and supporting documents to prove identity
Voters ID
NSO Marriage Contract
Land Title
Seamans Book
Elementary or High School Form 137 or Transcript of Records with readable dry seal
Government Service Record
NBI Clearance
Police Clearance
Barangay Clearance
Old documents issued at least one year prior to date of application that show correct
name, date and place of birth, picture and signature of applicant such as the following:
Voters Certification, List of Voters and Voters Registration Record (for newly
issued VRR, please attach receipt)
School Yearbook
Passport Fees
For First time passport applicants:
Please take note that there is an additional fee of P200.00 for a lost valid MRRP / MRP or
P350.00 for a lost valid e-passport.
You will be asked to provide your personal information for the passport application on the
online application form. Remember to fill in all the information slots correctly.

Please take note of the requirements that you need to bring to the DFA Consular Office
on your appointment date.

Choose an appointment DATE and TIME.
4.
Check your email for your reference number and a link to print your application form. You
will need to bring the printed application form to the DFA on your appointment.

New passport applicants will be asked for a copy of their NSO Birth Certificate. Please
make sure that you have one before your appointment date.

On your appointment date, proceed to the DFA Consular Office located at ASEANA
Business Park, Bradco Avenue corner Macapagal Boulevard, Paranaque City (see locator
map). You must be at the DFA Consular Office 30 minutes before your scheduled time.
Early/Late comers will not be entertained.
7.
Present your printed application form to the Appointment Counter.
8.
Get your queue number at the Information Counter and sit on the waiting section of the
Processing Area. Wait for your queue number to appear on the monitor indicating what
processing window you will go to.
9.
After processing, proceed to the Cashier's area located at the 2nd Floor. Pay the
applicable passport processing fees (see passport fees).
10. After payment, proceed to the Encoding Section for data capturing.
11.
After your photo is captured, personal details and biometrics are encoded, you have the
option of:
a) claiming your passport at the DFA consular office where it was applied at; or
b) have it delivered by courier to your home.
If availing of the delivery, proceed to the Delivery Counter to pay for the delivery fee.
