the bed can be a blessing and a spiritual
experience for all involved.

Dying at home should


be an option for all who
choose it.
There is a great divide separating the
kind of care Americans say they want
at the end of life and what our culture
currently provides. Surveys show that
we want to die at home, free of pain,
surrounded by the people we love. But
the vast majority of us die in the hospital,
alone, and experiencing unnecessary
discomfort.
It is unfortunate that many people
who have died in a hospital or who have
received heroic treatments to prolong life
in a hospital or nursing home may have
had the alternative of dying at home in

familiar surroundings, with family or other


loved ones at their side.
When someone is dying but there really
is no hope for recovery, the family often
calls 911 and starts a process that can
result in great stress and great emotional
discomfort. The loved one who is dying
ends up in a hospital or nursing home in
a strange environment, frightened and
confused and tied to tubes and monitoring
devices. This is not the ideal way in which
to spend ones last hours on earth.
The end of life cannot be predicted
for any of us. We do not know when it
will happen, who will be with us, how it
will occur, or what we will feel. However,
we do know that most people prefer to
die at home surrounded by their loved

Remember that not all people who are


dying are elders. Many are children.
Hospice is a form of medically
supportive care for patients who are
terminally ill. It allows for compassion
and dignity in the process of dying. A
commonly used definition for terminally
ill patients is, patients who have a
progressive, incurable illness that will end
in death despite good treatment, and who
are sick enough that you would not be
surprised if they died within six months.

state of Wisconsin can be found at
wisconsinhospice.org. The site has lots of
other useful information.
Hospice care is a valuable service and
is generally underused except for terminal

ones because it allows them to be safe,


comfortable and secure. As such, they are
dying on their own terms. Hospice can
allow this to happen. Memories of a loved
one passing in peace can provide great
comfort for family members in years to
come.
When there is no longer hope for
prolonging life, especially when this
decision is made months in advance,
hospice is a better alternative to other
medical intervention. Advances in
technology (home ventilators, tube
feeding) and greater availability of services
to support families at home (home care
and hospice services) have made it easier
for elders, children and families to choose
to die at home.

To assist the patient with the
emotional, psychosocial and spiritual
aspects of dying;
To provide needed medications,

To deliver special services like
speech and physical therapy when
needed;
To makes short-term inpatient care
available when pain or symptoms
become too difficult to manage at
home, or the caregiver needs respite
time; and
To provide bereavement care and
counseling to surviving family and
friends.

A person can receive hospice from


Medicare if he or she is:
Eligible for Medicare Part A
(Hospital Insurance), and

Hospice is a form
of medically supportive
care for patients who
are terminally ill.
The doctor and the hospice medical
director certify that the person is
terminally ill and probably has less
than six months to live, and
The person or a family member signs
a statement choosing hospice care
instead of routine Medicare covered
benefits for the terminal illness, and
Care is received from a Medicareapproved hospice program.
A person may continue to receive
regular Medicare benefits from his or
her customary doctors for conditions not
related to the hospice condition.

provided in patients homes (including
assisted living facilities and nursing

Medication safety tips for children and adults

Prescription and over-the-counter


medications can save lives and help people
of all ages manage certain conditions. When
used correctly and under the guidance of a
physician, medications are largely safe. Its
when medicines are used off-label, shared
or taken in error that reactions and injury
can occur.
The American Academy of Pediatrics
and their Healthy Children Organization
warns that more than 7,000 children visit
hospital emergency rooms every year for
problems related to medication errors.
Children are not the only ones in danger.

Adults can make mistakes with their


medications, as well. For example, seniors
who may be managing several different
types of medications can inadvertently
cause dangerous drug interactions by
mixing the wrong pills.
Pharmacists work diligently to help
prevent medication errors. However,
the general public can also do their part.
The American Society of Health-System
Pharmacists and the National Association
of Boards of Pharmacy
offer these
medication safety tips.

SKILLED NURSING

When a new medication is prescribed,


ask the doctor to explain more about it,
including its intended purpose and any
common side effects to be expected.
Make sure your doctor knows about
all the medications you are taking,
including non-prescription products,
herbal remedies, dietary supplements, and
vitamins. Some medications do not mix
with seemingly innocent ingredients. Keep
a running list of any medicines you take so
you can easily and accurately share this
information with your physician.
Question anything that you do not
understand. Check the prescription for
dosing information. For refills, make sure
the refill information conforms to the
original prescription strength.
Fill all prescriptions at the same
pharmacy and develop a rapport with
the pharmacist so that potential drug
interactions will be flagged. Pharmacists
are well versed in medications and may
be able to inform you as to the safety or
risk involved in taking an over-the-counter
product at the same time that you are on a
prescription.
Many pills look the same. If you are
confused and taking multiple medications,
keep medications in the original packaging
and double-check the labels before taking
any medications.
Use the right dosing tools. A spoon
from the kitchen is not accurate for
measuring out a teaspoon of medication.
If you take multiple medications, use
a pillbox to keep pills organized. The box
makes it easier to manage medications and
serves as a reminder if you have or have
not taken a medication on a given day.
Store medications as instructed on the
label. The bathroom medicine cabinet may
not be an ideal place to store medications,
as bathrooms get damp, and that can
compromise the integrity of the pills. Also,
bathroom cabinets are readily accessible
by all, including kids. Its better to store
drugs out of sight and reach of children.
Keep dangerous medications locked away.
Routinely discard expired or unneeded
medications. Medicine take-back programs
for disposal are a good way to remove
medicines from the home and reduce the
chance that others may accidentally take
the medicine.
Consult with a doctor before beginning
or ending medication.
Medicines play important roles in
personal health. When used correctly,
medications are assets, but caution should
always be taken to ensure safe usage and
storage of any medications.

Getting behind the wheel and enjoying


the freedom to travel is a luxury few are
anxious to abandon. But there comes a
time in nearly every persons life when he
or she must take inventory of his or her
driving and assess if that next joy ride is a
safe and smart decision.
Driving may help older adults remain
independent and mobile, but the chance
for a motor vehicle accident increases
as one ages. The Insurance Institute for
Highway Safety says fatal crash rates per
mile traveled increase starting at age 75 and
increase notably after age 80. This is largely
due to increased susceptibility to injury
and medical complications among older
drivers rather than an increased tendency
to get into crashes. Road accidents tend to
increase around a persons 65th birthday.

There is no set age when a persons keys


should be taken away, as aging does not
guarantee drivers abilities will decline. In
fact, there are many things people can do
to prolong their time on the road.
Get routine vision and hearing checks.
These examinations will help to determine
your fitness to be on the road and shed light
on any issues that need to be addressed.
Those who wear contact lenses or glasses
may need to update their prescriptions.
Get enough sleep. Lack of sleep can
affect the body in many ways, including
reducing reaction time. Falling asleep
while driving can compromise the safety
of drivers, their passengers and fellow
motorists.
Know your medications. Certain
medications can make you drowsy, so read
