Permit Application Due

Brenda:
The current permit for Pineview (and UIC regulations) requires PTP to apply for a new permit prior to the
expiration date of Sept. 7, 2009; "Duty to Reapply" Part III E.2., page 15. UIC regulations specify that your
application be submitted to EPA 180 days prior to the expiration date.......this is not specified in the current
permit, hence this message to you; 180 days from prior to expiration is (was) 7 March 2009.
Please have PTP submit a reapplication notice to EPA within the next 10 days specifying the number
wells it would like to operate, identifying the wells by the system used in the draft permit. Also, this notice
might want to mention PTP's interest/efforts related to transferring the responsibility of the subject wells to
the Pineview HOA.....see Part III.B.2. of the current permit. A link to the standard gov't. form for UIC
Permit Application is below. If you have any questions please feel free to call me. Thanks, Aaron

RE: FW: WETLAB results

Aaron,
I have a glimmer of hope. I spoke with Jim and he indicated that there is
nothing to indicate to him that the system is not functioning properly.
I asked the lab about the sampling and if there had been a change in who was
taking the samples. Andy Smith indicated that the sampler is now Genevieve
and has been for maybe about the same time. I have a call into the owner to
get his permission to get more testing ASAP and the lab is ready to go. Jim
Robinson will meet with Genevieve when she does her sampling to confirm she is
indeed testing from the proper inspection ports. The timing of the change in
personnel is suspicious.
I will keep you posted.

As soon as possible please obtain and analyze influent and effluent wastewater
samples for BOD, TSS and TNitrogen from Leachfields A and B..........results
from the last couple of months indicate that the treatment units are not
functioning properly. Leach Field A effluent samples are missing from the
reports, and I'm not sure if there is a port where effluent grabs can be
obtained for this field. Could you please look into this as the permit
requires an assessment of treatment efficiency for the units out there and the
only way to do this is to measure BOD, TSS and TNitrogen going in and coming
out.

Aaron,
Thank you for your last e-mail.

It was exactly what I was looking for.

Andy replied to my request last Friday, but my e-mail server was giving us
fits until yesterday. I spoke with Andy this morning and he indicated that
everything appears to be fine on their end (see below).
Would you like them to do another sample and test? He indicated running tests
now on the old samples may be problematic. He also said the tests are diluted
so maybe the dilution made a small issue seem larger.
Bottom line is it looks like the testing is not in error.
input on it.

Maybe Tom has some

Hi Brenda, I left a voice mail for Lyn yesterday but was unable to get in
touch with him.
I looked into the results/reports/calculations for the results in question and
found no issues. Reanalysis is always an option, however very much past the
holding times for BOD and TSS.
Please let me know how we should proceed or if you need anything further.
I will be available by email mostly today as I am out of the office.
However feel free to call my cell phone if you would like to discuss further.

Andy
