3. Work procedure.
* Confined space entry permit system shall be followed strictly before entry int
o the work area. * Supervisor / HSE * Proper ventilation by blower and lighting
no less than 53 lux no shadows arrangement shall be provided. * All vessel must
be ventilated and have a constant flow of moving air before a permit issued. * G
as test limit for a normal vessel entry are oxygen 19.5% , combustible gas at 0%
, and Hydrogen Sulphide at 0 PPM. * Gas test are not in normal range and their u
rgent work, respirators, protective clothing and forced air ventilation shall be
required. * The result of gas test and special precautions must be recorded on
the vessel entry permit and gas test sheet. * Gas test required and provided by
safety department as necessary.
* For Lifting arrangement 24V (Flame Proof) power supply or lighting only allowe
d with proper * Supervisor / HSE / Rigger grounding system. No 240V power supply
allowed at any case inside the confined space. * All worker should have PPE acc
ording to entry permit, additional if required. * Daily specific task Tool box m
eeting shall be conduct prior to start. * Daily checking list shall be prepared
by job supervisor to ensure the safe working condition and share the counter mea
sures whicg is involved in day activity. * Rescue system etc. to be place during
confined space working. * Appropriate safety sign / signals board to be provide
d during work for clear understanding to all of them who are working at MEG site
and PNCP in confined space. * Assigned one guide men to maintain the in and out
record for all persons who are working inside. Frequancy should be maintained i
n ragister. * Working areas to display sign stating "Danger" "Work in Progress"

4. Scaffold Dismantling
* No scaffold shall be dismantled, or altered except * Scaffolding Supervisor /
Rigger under the supervision of competent person. * Install specific ladder and
other temporary facilities in accordance with the work safety plan and IOCL/EIL
Safety rule & regulations. * All scaffold material to be lowered to the ground b
y rope only, not thrown down. * Dismantle the scaffold don't demolish it. materi
al to be inspect prior to use. Every week inspection required by competent perso
n or job supervisor and fill the green tag with remarks. * During dismantling ti
e of to separate and secured ledger that are locked in place. * All scaffold wor
k done by experience and skilled scaffold only. * Scaffold must not be Red Scaff
Tag has been issued and displayed on scaffold during dismantling. * Appropriate
PPE to be worn at all the time i.e. Helmet, safety shoe, Goggles, Full body har
ness, etc. and attached to a strong fixing point.

* All tools and tackles shall be inspected and approved by SECL Safety personnel

installation sequence starting from bottom the lower level tray manhole must be

side. Supervisor / HSE Frequency should be maintained in register. * Supervisor
should know the emergency contact numbers, assembly point and IOCL First aid cen
ter. * Standby vehicle required during working time.
