Our main goal is the successful and permanent placement of each dog we rescue into loving
forever homes. We strive to make sure you adopt a dog that is an age, size, temperament and
activity level match for your family and that the adoption is forever. We feel totally responsible
for all dogs we rescue, and we must guarantee their well-being and future. We want every dog to
go to its forever home with our blessing and knowledge that it is the best home for that dog.

Describe your home, yard, and fencing. Do you own your home? Please include pictures if
possible. How long have you lived at this address? Will you be moving within the next 6
months? My backyard has a 6 foot privacy fence. The backyard is around a quarter of an
acre. I own the home and do not plan on moving anytime soon. I have lived at this address
for nearly 2 years.
Describe your family composition. Include all people and pets in your home or family that
share your home any part of the time. Mention ages and personalities, likes, dislikes etc.
Are your pets spayed/neutered? Are they up-to-date on vaccinations? Please include
pictures if possible. If you have no children, are their children in your future? Do children
frequently visit your home? Does anyone have pet allergies? I live alone mostly with
roommates that come and go. I have 1 small dog which is spayed and up to date on her shots.
I have no children or plan on having any soon. Children come over to my house about once a
month. I have no pet allergies.
Tell about your past pets. Were they kept up-to-date on their vaccinations? Were they
spayed and neutered? My last dog was a German Shepherd who was neutered and up to date
on his shots.
Describe what you want this dog to do; such as go on walks, bark at strangers, play with
kids, obedience, etc. Who will have chief responsibility for the care of your new dog? Im
looking for a dog to play fetch with and to keep me company. I would like for the dog to bark

at unknown personnel, but not to actually attack them. Ill be the sole caretaker of the dog.

I have had no dogs that have had to live with someone else.
How do you plan to control external parasites? Ill use flea collars.
