ELGIBILITY Entrants must be a full member of a CDGA Affiliated Club. CDGA GHIN # must be
on application. Team participants must be full, non-restrictive golfing members of the club they Team #1
are representing.

FORMAT Field limited to 28 teams, one from each club. Any open remaining spots will be filled

No practice round is included in entry.
Team #2 this team will be put into a "draw" after deadline to fill field. Clubs will be notified of entry.

CADDIES The following constitutes a condition of the competition and is authorized by Appendix I,
Part C, Section 3 of the Rules of Golf. “A player is prohibited from having a parent, guardian or
another player in the same competition serve as his caddie during the stipulated round.” Entry Fee: $750 per team
TEE TIMES Requested tee times can not be guaranteed. Copies of starting times will be sent to Members of Jefferson, York or Columbus may enter this event for $675
CDGA Pro Shops approximately one week in advance of event. Contestants may also check (10% discount as a host club member)
OHIOGOLF.ORG for tee times.
Check made out to CDGA must accompany this applicaiton.

HAMILTON HOUSE
DEADLINE: May 4, 2010, 4 p.m.
The Columbus District Golf Association will send $5 per player from
each 2010 entry fee to the Capital Campaign for the new Evans
No Tee entries.
Scholar House at the Ohio State University. The new house, located Entry includes cart, range fee, and continental breakfast at each event.
at 52 East 14th Avenue is named the Hamilton House in honor of
Earl “Bones” Hamilton. $5 per player of the entry fee qualifies as a
charitable donation for federal income tax purposes.

Evans Scholars Foundation administers the nation’s largest privately Columbus District Golf Association
funded college scholarship program. Since 1930, over 8,500 1570 West First Avenue
deserving young caddies have earned their college degrees through Columbus, OH 43212
Evans Scholarships. Today, more than 820 caddies attend college
each year as Evans Scholars. The Evans Scholarship was founded by
golf great Charles “Chick” Evans Jr. (1890-1979).
